• Note (General): type:text; 本年度は、大迫町の住民に対して実施された検診にて、頭部MRI 撮影、頸動脈エコー検査、心電図検査、抑うつに関する調査、認知機能検査を行い、本研究に関するデータを収集した。新型コロナウイルス流行に伴い、2月後半以降の調査・打合せが延期となった。本年度延期された調査は次年度に実施する予定である。また当初予定されていた質問紙pilot調査が行政との調整により延期となった。 次に、これまで収集した既存資料を使用し、睡眠時間と認知機能との関連を検討した。睡眠時間と認知機能低下との関連について、多くの検討がなされているものの、その結果は一貫していない。一方、自由行動下血圧 (ABP)測定により得られる血圧値及び血圧短期変動と認知機能低下との関連が報告されている。そこで本研究では、ABP値及び血圧短期変動を考慮し、睡眠時間と認知機能低下との関連を検討した。岩手県花巻市大迫町の55歳以上の一般住民のうち、Mini-Mental State Examination (MMSE)及びABP測定を行った1113名 (平均年齢66歳、女性66％)を対象とした。睡眠時間はABP測定時の日記による自己申告から収集し、また認知機能低下は、MMSEが24点未満と定義した。1113名中189名で認知機能低下がみられた。ABP値・血圧短期変動(visit variability independent of the mean [VIM])を含む各種危険因子で調整した多重ロジスティック回帰分析を行ったところ、睡眠時間1時間増加ごとの認知機能低下となるオッズ比は、1.18 (p=0.02)であった。高血圧の有無による層別解析では、有意な交互作用は認められなかったものの、特に高血圧者において、睡眠時間は認知機能低下と関連していた。結論として、長時間睡眠はABP値・血圧短期変動と独立した認知機能低下の関連因子である可能性が示唆された。本研究結果は、次年度日本高血圧学会にて発表を予定している。 なお、本研究は慶應義塾大学薬学部倫理審査委員会（承191010-2、承191010-3）及び大迫町健康福祉課より承認を得ている。 Annual surveys including blood pressure measuring, MRI scan, carotid ultrasound, electrocardiography, depression test and examination of cognitive function were performed for the study subjects in Ohasama, Iwate. After late February, the annual surveys were postponed to the next year due to the COVID-19 crisis. The pilot questionnaire study was also postponed by the town. The association between sleep duration and cognitive function were examined in the existing data. Several studies have examined whether sleep duration related to cognitive decline, however, the results of obtained so far are controversial. No previous studies considered the effect of short-term blood pressure variability. Therefore, I investigated the association between sleep duration and cognitive function, considering the effects of ambulatory blood pressure level and short-term blood pressure variability. The Mini-Mental State Examination and ambulatory blood pressure monitoring were performed in 1,113 participants aged at least 55 years (mean age, 66.4 years old, 66% women) in the general Japanese population. Sleep duration was based on the self-reported diary at ambulatory blood pressure monitoring. Cognitive decline was defined as a score of 24 or less in the Mini-Mental State Examination. Multiple logistic regression model adjusted for diurnal systolic blood pressure level, visit-to-visit variability independent of the mean (VIM) as short-term blood pressure variability, and possible confounding factors was employed to assess whether sleep duration was associated with the cognitive decline. Of the 1113 participants, 189 had a cognitive decline. Odds ratios for the risk of cognitive decline were 1.18 (p=0.02) per 1-h increase in sleep duration. In stratified analysis by hypertension, significant associations were more clearly observed among hypertensive subjects, although there were no significant interactions. Long sleep duration might be a risk factor in cognitive decline independent from ambulatory blood pressure level and short-term blood pressure variability. These results will be presented at the conference of the Japanese Society of Hypertension. The study protocol was approved by the Keio University Faculty of Pharmacy ethics committee for research involving humans (No. 191010-2 and No. 191010-2) and by the Department of Health of the Ohasama Town Government.
