A strategy for the treatment of post cardiac arrest syndrome adopting the mechanism of remote ischemic preconditioning

遠隔虚血プレコンディショニングを伝達する液性因子であり虚血再灌流障害を軽減すると言われているキヌレン酸 (KYNA) が、心停止 (CA) /心肺蘇生 (CPR) 後の予後に及ぼす影響についてマウスモデルで検討した。CA/CPRから30分後の血清KYNA濃度は、生存群より7日以内に死亡したマウスで有意に高かった。CPR開始前にKYNAを投与するとCPR開始5分後の血圧は著明に増加したものの、血清過酸化水素濃度も著増し生存率は改善しなかった。KYNAによるCA/CPR後の活性酸素種産生増加や心筋の陽性変力作用のメカニズムの解明、KYNA産生抑制とCPR後の予後改善の可能性などさらなる研究が必要である。
Kynurenic acid (KYNA), one of the humoral mediators of remote ischemic preconditioning, has been reported to protect organs from ischemia reperfusion injury. We studied the impact of KYNA on the outcome of cardiac arrest (CA) and cardiopulmonary resuscitation (CPR) in mice. Serum KYNA level at 30 minutes after CPR was markedly increased in the dead mice group after CA/CPR compared with the alive. Administration of 80 mg/kg KYNA 1 minutes before CPR increased mean arterial pressure but also serum hydrogen peroxide level at 5 minutes after CPR, and then failed to improve survival rate after CA/CPR. Further study is needed to reveal the mechanism responsible for the cardiac inotropic effect and upregulation of reactive oxygen species production.
