Development of an outdoor education activity program to support pediatric liver transplant children during the transitional period

1989年、わが国で最初の生体肝移植が行われてから、多くの子どもたちが成長発達を遂げた。しかし、アドヒアランスの低下によるグラフト不全や原疾患の再発により、再移植や再々移植が報告されている。そこで、小児から成人期への移行期の支援体制として、野外教育活動を構築した。
2015年から3年間、スキーを中心とした野外教育活動を行い、17名が参加した。生きる力尺度（IKR尺度）を用いて、事前、事後、追跡（1か月後）の変化を評価したが、有意差はみられなかった。しかし、保護者からは、事後に見られた子どもの行動変容についての回答を得られた。
Since the first living-donor liver transplantation was performed in Japan in 1989, many children have grown and developed. However, re-transplantation and re-transplantation have been reported due to graft failure due because of non-adherence and recurrence of the underlying disease. Therefore, we have established an outdoor education activity as a support system for children with liver transplantation during the transition period from childhood to adulthood.
From 2015 to 2018, we conducted outdoor education activities (mainly skiing), and 17 children participated. Changes in pre-, post-, and follow-up (1 month later) were assessed using the zest for life scale (IKR scale), but no significant difference was found. However, parents answered that the children had changed their behavior after attending outdoor education activities.
