- <p>Background</p><p>The prevalence of peanut and tree nut allergies in Japan has increased. However, information on sensitization in high-risk infants with food allergies (FA) remains limited. Therefore, we examined the sensitization rates of peanuts, walnuts, and cashews among children aged 0-2 years with FA.</p><p>Methods</p><p>Children aged 0-2 years who had FA between January 2019 and February 2022 were included in this study. Patient background, crude antigen and component-specific IgE levels, and sensitization rates were retrospectively evaluated from medical records.</p><p>Results</p><p>Of the 237 participants, 200 were evaluated for at least one crude antigen. Peanut sensitization was observed in 50 of 190 subjects (26%), walnut sensitization in 26 of 158 subjects (16%), and cashew sensitization in 35 of 107 subjects (33%). Peanut and walnut sensitization rates increased with age. Multivariate analysis revealed that age and egg allergy were the factors involved in peanut sensitization, age at walnut sensitization, and male sex in cashew sensitization.</p><p>Conclusion</p><p>Sensitization rates to peanuts, walnuts, and cashews in children aged 0-2 years with FA are not low and should be noted in practice.</p><p>【目的】食物アレルギーが疑われた0~2歳児におけるピーナッツ,クルミ,カシューナッツの感作率を検討した.</p><p>【方法】2019年1月~2022年2月に食物アレルギー診療のため受診した0~2歳児を対象とした.後方視的症例集積研究として,粗抗原およびコンポーネント特異的IgE値,その感作率を診療録より評価した.</p><p>【結果】対象237名のうち200名が少なくとも1つの粗抗原の評価を行った.ピーナッツ感作を190名中50名(26%),クルミ感作を158名中26名(16%),そしてカシューナッツ感作を107名中35名(33%)に認めた.ピーナッツ,クルミの感作率は年齢が上がるにつれて増加していた.多変量解析により,ピーナッツ感作には年齢と鶏卵アレルギー,クルミ感作には年齢,カシューナッツ感作には男児が関与する因子であった.</p><p>【結語】食物アレルギーを有する0~2歳児におけるピーナッツ,クルミ,カシューナッツに対する感作率は低くなく,診療において注意する必要がある.</p>
