- <p>Dextromethorphan (DXM) is a non-narcotic, centrally acting antitussive agent with a long-standing clinical history. Due to its <i>N</i>-methyl-D-aspartate (NMDA) receptor antagonism, DXM can induce euphoria and mood elevation, which has contributed to its abuse for many years. In Japan, its approval as an over-the-counter (OTC) medication in 2021 has been associated with a marked increase in misuse, particularly among adolescents. DXM also possesses serotonin-norepinephrine reuptake inhibitory (SNRI) properties, and several reports have linked its use to serotonin syndrome.</p><p>We present a case of serotonin syndrome in an adolescent female who had been prescribed escitalopram and trazodone and ingested a large quantity of DXM. Awareness of DXM’s SNRI effects facilitated a prompt and accurate diagnosis, allowing early initiation of treatment.</p><p>OTC drug abuse is currently spreading among Japanese youth, with DXM playing a central role in this trend. Psychiatrists, especially those working in general hospitals, should be familiar with the pharmacological mechanisms of DXM and maintain a high index of suspicion for serotonin syndrome in cases of suspected DXM abuse.</p><p>デキストロメトルファン(dextromethorphan,DXM)は古い歴史を有する非麻薬性の中枢性鎮咳薬である。<i>N</i>-methyl-D-aspartate(NMDA)受容体拮抗作用を有し,多幸感や気分高揚感等を引き起こすため,以前から乱用されていた。本邦では2021年にDXMは処方箋の不要なover-the-counter drug(OTC薬)として認可されたことから,その乱用が流行している。またDXMはserotonin-norepinephrine reuptake inhibitors(SNRI)作用も有しており,その使用でセロトニン症候群が発生すると報告されている。今回われわれはエスシタロプラム,トラゾドン内服中の10歳代前半女性がDXMの大量服薬によりセロトニン症候群を発症した症例を経験した。われわれはDXMがSNRI作用を有していることを理解していたことから,迅速かつ的確にセロトニン症候群と診断し治療を開始することができた。現在,若年層でOTC薬の乱用が流行しており,DXMはその中核をなしている。そのため総合病院精神科医師はDXMの作用機序について十分に理解し,DXM乱用時にはセロトニン症候群の発生にも十分に留意すべきである。</p>
