Note (General): 【目的】本研究の目的は,急性期脳梗塞患者に対する歩行中の機能的電気刺激(functional electrical Stimulation:FES)が運動学的・運動力学的歩行パラメータおよび内側感覚運動皮質(medial sensorimotor cortices:mSMC)のヘモグロビン濃度に及ぼす即時的影響を検証することである。【方法】対象は発症後 14日以内の脳梗塞患者8名とし,FESは遊脚期に前脛骨筋を,立脚後期に腓腹筋を刺激した。歩行パターンの変化は三次元動作解析システムを用い,mSMCのヘモグロビン濃度変化は近赤外線分光法を用いて計測した。【結果】FES実施前と比較してFES実施中に,歩行速度や,麻痺側立脚期の股関節屈曲モーメント,膝関節伸展モーメントおよび足関節底屈モーメントが有意に上昇し,FES終了後にも持続した。また, FES実施中の非損傷側mSMCの酸素化ヘモグロビン濃度は, FES非実施時と比較して加速期・定速期とも有意に低下していた。【結論】FESは麻痺側立脚期の支持性向上や麻痺側立脚後期のrolLoff機能向上といった歩行パターンを変化させ,さらに非損傷側mSMCの過剰な脳活動を抑制している可能性が示された。
