Note (General): 近年の文献報告では、バンコマイシン(以下、VCM)の有効性は24時間の血清中濃度時間曲線下面積(以下、AUC_24)と最小発育阻止濃度(以下、MIC)の比(以下、AUC_24/MIC)とよく相関し、methicillin-resitant Staphylococcus aureus(以下、MRSA)肺炎においてはその値が400以上必要と報告されている。我々は、現在のVCM治療域濃度の妥当性を評価するため、トラフ濃度とAUC_24との関係、さらにAUC_24/MICの要素であるMICについて調査した。福井大学医学部附属病院においてVCMが投与された成人患者188名を対象とし、ベイジアン法を用いて求めた推定動態パラメータから定常状態におけるAUC_24を算出した。その結果、トラフ濃度と算出したAUC_24は、y=25.60x+124.39の回帰式で示された。得られた回帰式、並びにMRSA臨床分離株のMIC分布(MIC_50 1.5μg/mL, MIC_90 2μg/mL)から考えると、AUC_24/MIC>400を満たすためには、トラフ濃度を15～25μg/mL以上に保つ必要があると考えられた。
