症例は73歳、女性。非小細胞肺がん（Stage ⅣA）に対してPD-L1免疫染色高発現であったため1次治療としてペムブロリズマブ（pembrolizumab）投与を開始した。その後、円形びらんと水疱が出現し、皮膚生検にて水疱性類天疱瘡の診断となった。ペムブロリズマブの休薬と全身ステロイド投与にて皮疹は改善した。ステロイドは漸減し少量維持投与を継続しているが、皮膚所見の再燃なく抗腫瘍効果も持続し、ペムブロリズマブ中止後2年半以上の無増悪生存期間を得ている。水疱性類天疱瘡は稀な免疫関連有害事象であるが、早期発見し治療を行うことが重要であり、また、水疱性類天疱瘡発症症例での予後について、今後、抗腫瘍効果、生存率などのデータ蓄積が必要である。
