食道扁平上皮癌におけるCCR7発現に関わる基礎的検討及び臨床的意義について検討した。当院で根治手術を行った術前未治療胸腹部食道扁平上皮癌患者105名(表在癌は61名)に対し、免疫組織化学的手法を用いて解析を行った。基礎的検討では、ヒト由来食道扁平上皮癌細胞株TE 10種を用いて、real time RT-PCR、adhesion assay、リンパ節転移モデルマウスを用いた転移解析を行った。免疫組織化学ではCCR7陽性例は28例(26.7%)、うち23例(82.1%)が転移陽性(p=0.04)、OS及びRFSも有意な差を認めた(OS/RFS: p=0.02/p0.01。T1症例では、陽性例は17例(27.9%)、うち10例(58.9%)が転移陽性(p0.01)。OS・RFSにおいても有意な差を認めた(OS/RFS: p=0.04/p0.01)。RT-PCRでは、10種のTE全てに様々なレベルでの発現を認め、最も発現の少ないTE4及びTE4にCCR7を過剰発現させたTE4CCR7+を用いてadhesion assayを行ったところ、TE4CCR7+はTE4に比して内皮細胞に対する有意な接着能の亢進が認められた。また、リンパ節転移モデルマウスでは、TE4CCR7+はTE4に比してより早期に転移することが分かった。これらの結果から、CCR7は食道扁平上皮癌リンパ節転移における重要な役割を担っている可能性が示唆された。
