胸部下行・胸腹部大動脈遮断時の脊髄保護を目的として、脊髄冷却用の硬膜外カテーテルを開発した。本カテーテルは内部にCountercurrent lumenを有し、inletから注入された冷却水がカテーテル先端で反転しoutletから排出されるしくみである。外部回路と接続し閉鎖回路内に冷却水を循環させた。本法の有効性は既に動物実験で証明した。今回、事前同意を得た26症例に対し、本法を臨床応用した。在院死亡が1例あったが、術後脊髄障害や出血傾向などの合併症はなかった。今後、症例数を増やして臨床的有用性に関しさらなる検討が必要であるが、胸部下行・胸腹部大動脈瘤手術を安全に行うための一助となる可能性があると考えられる。
