副腎は胎児と成人では形態学的にも内分泌学的にも著しく異なった特徴をもつ。そのため新生児、殊に早産児では正確な副腎機能評価が困難であった。新生児の副腎機能を評価するにあたり最も信頼性の高いLCMSMS法により血中ステロイド一斉測定を行い、早産児と正期産児では出生後のステロイド分泌パターンは同様である事、在胎週数が若いほど胎生皮質である170Hプレグネノロンが高値になるだけではなく、170HPも高値となる事が示された。また、これらの状態にも関わらずコルチゾール値は大きく変わらないことから早産児の副腎機能が低いことが判明した。
