腸管の慢性炎症性疾患であるクローン病の原因は未だ解明されていない。一方で、好中球NADPH oxidaseの先天性異常のために、好中球殺菌機能が障害される慢性肉芽腫症において、クローン病類似の消化管病変を発症することが知られている。このことは、好中球殺菌能の異常により、クローン病に類似した病態が発症し得ることを示している。そこで、好中球による腸内細菌に対する殺菌能の異常がクローン病の病態形成に関与している可能性を考え、クローン病患者の好中球機能異常を系統的に調べ、クローン病の病態への関与を明らかにすることを目的として下記の検討を行った。クローン病患者の末梢血好中球および腸管粘膜内好中球を用いて、1) 細菌刺激によるサイトカイン産生能、2) 細菌貪食能、3) 活性酸素産生能、4) アポトーシス、5) 抗菌ペプチド産生能について検討した。アポトーシス、細菌貪食能、活性酸素産生、抗菌ペプチド産生は健常人の好中球と差を認めなかったが、クローン病患者の好中球からのIL-6・IL-1
