EGFR変異陽性肺癌にはEGFR阻害剤が著効するが、多くは1年以内に増悪(耐性化)するため、その克服が重要である。細胞株を用いた検討により、これまで報告されていない繊維芽細胞増殖因子(FG)および受容体(FGFR)が、EGFR阻害剤耐性に関与することを明らかにした。臨床検体では、FGF9高発現が肺癌患者の予後に関係することを見出した。FGFを肺上皮細胞に導入すると形質転換し、免疫不全マウスでの腫瘍形成が促進し、FGF阻害剤によりそれが抑制された。FGF/FGFR経路が肺癌の予後・腫瘍形成・耐性化に関与しており、治療標的となることを、細胞株から臨床検体レベルまでにおいて系統的に明らかにした。 Epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKIs) have great effect in the treatment of lung cancer patients with EGFR mutation. But many of the patients eventually get worsened (become 'resistance') within one year. It is important to overcome the 'EGFR-TKI-resistance'. Using lung cancer cell lines, we had elucidated that fibroblast growth factors (FGFs) and receptors (FGFRs) had related to the EGFT-TKI resistance. Using clinical specimens, high FGF9 expression is related to poor prognosis in lung cancer patients. Overexpression of FGF in lung epithelial cell induced transformation, and the tumor formation in immune-deficient mouse was accelerated. FGFR-inhibitor blocked that tumor formation. Using cell lines and clinical specimens, we have comprehensively revealed that FGF/FGFR pathway has an important role in prognosis, tumor formation, and resistance in lung cancer, suggesting that FGF/FGFR pathway can be possible drug target.
