重症患者は, 感染症が契機となって多臓器不全で死亡することが多い。宿主免疫能評価方法の一つであるプロテインアレイ法による白血球表面抗原の網羅的解析システムは重症患者においても迅速・簡便に行うことが可能であった。臓器障害発症前後に網羅的解析を行うことで, 臓器障害発症後に白血球表面抗原のうちCD4, CD36, CD16b, CD66bが有意に変化することが分かった。敗血症性ショック患者では臓器障害の重症度に反比例しCD4は直線的に減少した。白血球表面抗原の解析により臓器不全発症の予測が可能となる可能性が示唆された。 Critically ill patients often die of multiple organ failure following infection. Immunophenotyping (IP) method, a novel analysis system of diverse leukocyte surface antigens using the protein array method for evaluation of host immunological status, was sufficiently implemented in timely manner by bedside in critically ill patients. After the onset of organ failure, CD4 and CD36 decreased, CD16b and CD66b increased, respectively. Proportion of CD4 and the severity of the organ failure were inversely related in septic shock patients. The rapid bedside analysis of diverse leukocyte subsets with our IP method was applicable and could be useful for predicting organ failure in critically ill patients.
