本研究は我々の心嚢内視鏡技術により得られた画像を3次元マッピング情報として付加した新たなカテーテルアブレーションガイド法を可能にすることを目的としたものである。心臓モデルの周囲から撮像した画像をCADデータに構築し, 3Dプリンターで再現することに成功した。これらを心嚢内視鏡に搭載すべく, 内視鏡サイズ, 侵襲度, 解像度など様々な条件で最も適した仕様を検討した。イヌを用いた急性期慢性期検討では特記すべき合併症を認めなかった。画像解析時に必要な構造識別のための解剖学的特徴を検討し, 狭い心嚢腔での心臓全体を撮像することを課題とし, 現行の3次元マッピングシステムへの統合へ向けて今後も開発を行う。 This study was undertaken to develop a new modality of catheter ablation using the three-dimensional mapping data obtained from our pericardial endoscopic technology. Images filmed from the entire periphery of a cardiac model, which were converted to CAD data, were successfully printed out with a three-dimensional printer. We examined the proper condition for equipping this technology with our pericardial endoscopic maneuver regarding the size of the endoscopy, invasiveness, and resolution. In a canine experimental model, there were no major acute or chronic complications. Hereafter, we will focus on issues of the anatomical characteristics for the image analysis and its adaptability for capturing the whole image of the heart in the narrow pericardial space. We hope we can develop this technology as a pericardial endoscopy-guided catheter ablation system.
