本研究は, 水素吸入療法の院外心停止蘇生後患者に臨床応用を目指したトランスレーショナルリサーチである。まず, ラット心停止蘇生後モデルを用い水素吸入の有用性の検証した。水素吸入群では, 脳機能スコアおよび生存率が対照群と比較して改善した。水素吸入と低体温療法を併用することにより最も改善効果を認めた。さらに, 蘇生後患者に対し水素吸入の安全性と有効性の評価を行った。事前に設定した予測しうる心停止発症後7日間以内の臨床的異常変動を観察し, 副作用および臨床上の不利益をみとめなかった。対象全5例中4例が独歩退院した。今後, 多施設無作為化試験により心停止後症候群患者に対する水素吸入の効果を検証する予定である。本研究は, 水素吸入療法の院外心停止蘇生後患者に臨床応用を目指したトランスレーショナルリサーチである。まず, ラット心停止蘇生後モデルを用い水素吸入の有用性の検証した。水素吸入群では, 脳機能スコアおよび生存率が対照群と比較して改善した。水素吸入と低体温療法を併用することにより最も改善効果を認めた。さらに, 蘇生後患者に対し水素吸入の安全性と有効性の評価を行った。事前に設定した予測しうる心停止発症後7日間以内の臨床的異常変動を観察し, 副作用および臨床上の不利益をみとめなかった。対象全5例中4例が独歩退院した。今後, 多施設無作為化試験により心停止後症候群患者に対する水素吸入の効果を検証する予定である。 This study is a translational research aimed at clinical application of hydrogen inhalation therapy (HIT) in out-of-hospital cardiac arrest (CA) patient. First, we verified the usefulness of hydrogen inhalation using a rat CA model. Brain function scores and survival rates in the HIT group were improved compared to the control group. It showed the most improvement effect by the combined use of HIT and therapeutic hypothermia. In addition, we evaluated the safety and efficacy of HIT for patients successfully resuscitated after CA. There were no side effects and clinical disadvantages. Four patients of all 5 eligible patients discharged from hospital with alert consciousness. We are planning to verify the efficacy of HIT for post-CA patients by multicenter randomized trial.
