本邦で施行された冠動脈インターベンション手技(PCI)を米国で策定された適切性基準(AUC)で評価した場合, 米国での結果と同様に, 緊急PCIは概ね適切な適応のもとで施行されていた。一方, 待機的PCIに関しては, 2009年の基準では15%のPCIが不適切であると評価され, 改定版の2012年基準では, 30.7%ものPCIが不適切であると判定された。続いて, 各種の評価方法の施行率および適応適切性の経時的な変化を示したところ, 不適切なPCIの割合が年次推移とともに増加し, その傾向は冠動脈CTおよびFFRの増加と相関し, 負荷心筋シンチの減少と逆相関していることが明らかとなった。 The aim of this study was to evaluate the appropriateness of percutaneous coronary intervention (PCI) in Japan and clarify the association between trends of pre-procedural noninvasive testing and changes in appropriateness ratings. In this contemporary, multicenter Japanese PCI registry (JCD-KiCS), almost all acute PCIs were acceptable regardless of the criteria applied. However, approximately one-sixth of nonacute PCIs were rated as inappropriate under the original criteria. Under the updated AUC/2012, the rate of inappropriate PCIs increased to nearly one-third because noninvasive stress testing was not performed before a large number of elective PCIs in Japan and was seemingly affected by the increasing trend of coronary CT angiogram.
