生物学的製剤開始直前の関節リウマチ患者の血清中のオステオポンチン濃度が低値であると, TNF阻害薬のインフリキシマブではなくIL-6受容体阻害薬であるトシリズマブの1年後の反応性が良好であることを見出した。また, トシリズマブによる治療1年後まで経時的に血清中オステオポンチンを測定した結果, 濃度の変化はなかったため, IL-6よりも上流にオステオポンチンが位置することが推察された。トシリズマブによるIL-6受容体阻害でも関節リウマチの病勢が抑えられない患者が存在することから, この場合ではIL-6よりもオステオポンチンが寄与する病態になっている可能性が考えられた。 The study revealed that the low serum levels of osteopontin at baseline were related with the good response to tocilizumab (anti-interleukin-6 receptor antibody) but not to infliximab (anti tumor necrosis factor-alpha antibody) at 1 year in biologics-naive patients with rheumatoid arthritis (RA). The serum levels of osteopontin did not significantly change from baseline up to 1 year in patients with RA treated with tocilizumab, which suggested that osteopontin may be located upstream from interleukin-6.
