本研究の目的は, アトピー性皮膚炎, HIV/AIDS, 発達障害, 境界性パーソナリティ障害の4つの疾患を扱う医師を対象に, それぞれの疾患や患者のもつ「不確実性」にどう対処しているのかをインタビューをもとに明らかにすることである。調査の結果, 疾患の種類や治療法の確立の程度によって, それぞれの疾患をみる医師の直面する不確実性は多様であることが明らかになった。しかし, どの疾患においても, 患者の心理的な背景や人間関係といった, 診察室の外側で起こる事柄が不確実性のひとつの要素として語られていた。この点は, いくらガイドラインに基づく治療やEBMを徹底させたとしても解決されることのない問題であると考えられる。 The aim of this research is to describe how doctors deal with 'uncertainty' regarding treatments. Ushiyama, Shingae, Teruyama and Yoshida conducted interviews with doctors who treat atopic dermatitis, HIV/AIDS, developmental disabilities and borderline personality disorder respectively and compared the differences of the quality of 'uncertainty' depending upon the disease. It was found that doctors dealt with different type of 'uncertainty,' depending upon the type of disease and how well established the treatments are. However, in terms of each disease, doctors stated that aspects of patients' social life, such as their mental state and their relationship with family, friends or lovers, as an 'uncertainty.' Even though doctors ensure that they themselves follow the treatments based on guidelines and evidence-based medicine, such uncertainty cannot be solved and has to be dealt with doctors' skills and knowledge they have learnt tacitly in their clinical experiences.
