我々は, 臨床サンプルから永続的に増殖可能なオルガノイドを作製し, 希少がんの研究プラットフォームを開発した。更に, 構築された潰瘍性大腸炎患者関連大腸がんオルガノイドと通常型大腸がんの遺伝子変異・遺伝子発現変化の統合的な解析を行い, 潰瘍性大腸炎患者関連大腸がんに特異性を有する分子マーカーの探索を行った。大腸内視鏡解析や病理組織解析を組み合わせ, オルガノイドーオミックス解析で得られた分子マーカーを潰瘍性大腸炎患者関連大腸がんの診断技術向上に導く研究システムが開発された。潰瘍性大腸炎患者の更なる高齢化に伴い, 潰瘍性大腸炎患者関連大腸がんの増加が予想されており, 本研究成果は本邦において非常に有用である。 The number of patients with ulcerative colitis-associated colon cancer (CAC) has been increasing along with aging of patients with ulcerative colitis (UC) that became common diseases in the past decades. However, due to its rarity, it has been difficult to promote the research using human clinical samples. Thus, we have developed a research platform for rare cancer from clinical samples using establishment of organoids library that can be expanded indefinitely on demand. Furthermore, integrated analysis of genetic mutation and gene expression profile using the established sporadic colon cancer organoids and CAC organoids would enable the identification of a specific biomarker for CAC. Clonally amplified organoids enabled to perform next-generation sequencing of CAC regardless of original sample size. In particular, we developed a system which can improve the diagnostic technology of CAC by applying molecular biomarkers obtained from organoids to endoscopic and pathological analyses.
