【背景】1989年, わが国で最初の生体肝移植が行われてから多くの患児が成長発達を遂げ, 思春期・青年期を迎えている。この時期は, 特に身長・体重の増加という量的な変化に加え, 性的な成熟を獲得していく質的な変化をも伴うという, 生涯の中で最も身体的変化の大きい時期である。移植を受けた子どもたちは, 腹部に残る大きな手術瘢痕のことをどのように認識し, 日常生活を送っているのだろうか。本研究の目的は, 子どもたちの健全な成長発達を促すために, 腹部の手術瘢痕をどのように認識し, 実際の日常生活においてどのような問題があるのかについて明らかにすることにより, 長期ケアを提供するための示唆を得ることである。【方法】生体肝移植を受けた子どもたちの, 腹部手術瘢痕の知覚を量的に評価し, 腹部手術瘢痕に対する思いや日常生活における問題点について調査する。対象は, A大学病院小児外科外来へ通院する肝移植後患児(小学校4年生以上)で, 代諾者と本人の両方が, 本研究の目的と方法を説明を理解した上で, 参加に承諾が得られた10名(男児5名, 女児5名)とする。方法は, 腹部手術瘢痕の知覚については身体描画法により本人の知覚を測定し, 実際の瘢痕の大きさと比較する。また, 腹部手術瘢痕に対する思いや日常生活における問題については90分前後の半構成面接法を用い, 本人の語りを録音したテープから逐語録を作成し内容を分析する。【結果】平成29年度は, 主に文献レビューと海外の研究者と意見交換を行った。また, 患者会を開催し, 患児や家族の自由記載のアンケートから腹部手術瘢痕についての認識や日常生活や学校生活に対する影響について情報を得た。【事業計画】平成30年度は, これらの情報に基づき, 研究計画書やアンケート用紙を作成し, データ収集および分析方法をブラッシュアップし, 実際にデータ収集を行う予定である。 In 1989, the year of our first living transplantation was performed, many children have been growing up healthily and going through puberty, a very impressionable time in their lives, and some have troubles or difficulties in their daily life. The purpose of this study is to investigate how liver transplant children perceive their abdominal scars and the effects these scars have in their daily lives. In the year of 2017, the researcher reviewed related literature and had meetings with the research members and researchers of this field abroad. Also, the researcher arranged a patients' reunion and talked with some patients and their parents regarding their abdominal scars. In the year of 2018, the research is planning to write research proposal and questionnaire and collect data.
