医薬分業は, 医師による処方・調剤を, 医師による処方と薬剤師による調剤に分離することによって二重チェックを行い, 医療安全を高める制度である。欧米では当たり前のこの制度は, 日本では漢方医学の伝統と医薬品の差益(公定薬価と実勢価格の差)に頼った医療機関経営があったため, 長らく実現しなかったが, ここにきて日本全体で分業率が7割を超えるようになった。本研究は, 医薬分業を定性的・定量的に政策評価する。定性的には, 医薬分業自体の必要性については, 論を待たない。しかし, 日本の国民医療費は40兆円を超え, そのうち薬剤費が10兆円を占める中で, 医薬分業は調剤医療費の伸びの第一原因であり, 分業の効果が患者には実感できないことから, 医薬分業の制度的意義に疑問が生じている。厳密な費用対効果評価の実施が望ましいが, これはかなり困難である。より簡易な評価としては, 診療報酬における調剤報酬に見合ったサービスが提供されてはいないとの意見が大半であり(規制改革推進会議など), 平成30年度診療報酬改定ではその見直しが行われ, 薬剤師の本来的な業務である服薬管理の実施と地域におけるかかりつけ薬剤師の機能強化が行われた。今後もその方向での改革が続くと予想される。定量的には, 医薬品卸から実取引パネルデータを入手し分析を試みた。薬剤取引量に関する薬価差益弾力性を計測したところ, 一部薬剤で, また診療所で価格弾力性が高いことが判明した。弾力性がゼロでないということは, 薬価差益追求行動によって, 処方ないし調剤に歪みが出ていることを意味する。また, 比較的分業の進んでいない診療所で弾力性が高いという結果は, 分業の意義自体を確認することになった。ただし, 実取引データにおいて, 分業の有無を確認することは, あまりに煩雑なため実施できなかった。つまり, 分業ダミーを変数として投入することはできなかったため, 結果の解釈には注意を要する。 The separation of dispensary from medical practice (SDM) is common in European countries but not so in Japan until recently. Due to the rapid increase of dispensary-related medical costs, the separation itself has been questioned. This study evaluated the SDM from qualitative and quantitative standpoints. In the Japanese social insurance reimbursement system, lots of economical incentives have been given, but it is a consensus among experts and policy makers that pharmacists did not live up to the expectation. As expected, the reimbursement was lowered to encourage their activities in the last reimbursement reform. The researcher also collected real trading data of pharmaceuticals and analyzed them with panel-data analyses method. The finding is that the elasticity of trading quantity by the difference between the real price and reimbursed price (DRR) is not zero, meaning that there is a bias in dispensary, and the number is higher among clinics which have relatively low rate of the SDM. The limitation of the study was discussed.
