• Note (General): type:text; 介護保険制度が広く浸透された今日においても, 家族が担っている介護の負担は大きい。本研究者が昨年に行った調査結果によると, 介護の中で家族介護者が一番多く行っているのは「排泄に関する一連の介護」であった。排泄は, 生理的に重要であるのはもちろんであるが, 療養者と家族介護者の双方が特に気をつかう内容でもある。また, 夜間の介護が続くと介護者の疲労が蓄積し, 昼間の生活状況にも影響していた。 そこで, 本研究では, 在宅療養者が可能な限り希望する生活を継続するためには, ケアマネジメメントの要であるケアマネジャーが, どのように排泄の介護に関する情報を収集し, 各関連職種と連携してケアプランを提案しているのかを明らにするために, 居宅介護支援事業所に勤務する主任ケアマネジャーにインタビューを行った。 調査の結果, 主任ケアマネジャー4人の年齢は40代1名, 50代3名で, 性別は女性は3人, 男性1名で, ケアマネジャーになる前の職業は介護福祉士3名, 看護師1名であった。主任ケアマネジャーの経験年数はどの方も5年以上で, 1名の方は, おむつフイッターという資格をもっていた。 また, 3名の勤務する居宅看護支援事業所は, 訪問看護や訪問介護の事業所が併設されていたが, 1名は本人しかいない1人居宅介護支援事業所で, 他に併設している介護サービス事業所もなかった。 どのケアマネジャーも排泄について情報収集するときは, これまでの療養生活や介護者との関係性, さらにケアマネジャーとの信頼関係を維持できるように注意してタイミングを見ながらコミュニケ―ションをとっているとのことであった。さらに在宅生活を始める前から入院中から介護者の介護方法等を指導してほしいと話されていた。 今後は, インタビュー内容の分析を進め, 関連学会に発表する予定である。 Today, the long-term care insurance system is spreading, but the burden of family care is great. According to the findings discovered last year by researchers, caregivers were taking the "care of excretion" most. Both the pacient and the caregiver are using mind. Of course, excretion is physiologically important. In addition, as nighttime care continued, fatigue of caregivers accumulated and affected the living condition of the day. Therefore, the care manager which is the center of care management gathers information on excretion care about how to continue the life desired by home carer as much as possible in this research, contacts each facility and plans I investigated whether it stands. One in 40's, two in 50's, one in 60's, one three men in women. Everyone was a chief care manager for over five years. Also, one was qualified as a diaper fitter. When as a result of having interviewed it, and all people gathered information, I seemed to call while warning you so that the relationship with character and the caregiver of the person of medical treatment maintained a relationship of mutual trust with the Care manager more, and watching a timing. Since before having begun at-home life that is if I was admitted to the hospital, the Care manager of the one was told that I wanted you to teach the care methods of the caregiver from all over the hospitalization. I push forward the analysis of interview contents and am going to announce it in future in the associated society.
