本研究は, がん化学療法による皮膚障害のセルフマネジメントに対する自己効力感を測定するための尺度を作成し, 臨床における適用を目指して, 信頼性, 妥当性を検証することを目的とした。 ①仮尺度の決定 2016年度に実施したパイロット調査の結果について, 項目分析, I-T相関の確認, 項目同士の相関係数の算出, 探索的因子分析を行った。その結果に基づき, 13項目を削除, また一部の項目について質問の表現を変更し, 19項目から成る尺度案を決定した。 ②本調査の開始 2017年11月より本調査を開始した。研究への協力を依頼している大学病院において, 皮膚障害が高頻度に発生する抗がん剤, あるいは分子標的薬を含むレジメンで治療を受けている患者に対して調査を依頼し, 同意の取れた患者を対象とした。まずは一般消化器外科の乳腺外科外来で, 乳がんの治療を行っている患者への調査を開始した。 ③調査の進捗状況・今後の予定 2018年3月末時点で, 20名の対象者にアンケート(尺度案含む)への回答を依頼し, 19名より協力が得られた。手足症候群や皮膚乾燥症等, 皮膚障害を有する対象者は10名いた。調査対象には術後補助化学療法を目的とした患者も含まれていた。再発・進行がんなど化学療法以外に治療の選択肢がない状況にある患者と, 治療やセルフマネジメントの意味合いが異なるため, 今後は再発・進行がんの患者を対象として, 引き続き, 調査を進めていくこととする。女性の場合, 日常的なスキンケアとして保湿, 日焼けや水仕事による皮膚への刺激に対処することに取り組んでいる対象者も多く, セルフマネジメントのレベルが高い対象者も多く見られた。そのため, 今後は男性も対象とし, より高頻度に皮膚障害が発生するレジメンによる治療を受けている患者への調査が必要であるため, 胃がん, 大腸がん等の消化器がん, および肺がんの患者に対する調査を計画的に進めていきたいと考えている。 The purpose of this research is development the scale speccifically for measuring self-efficacy for self-management of skin reaction induced by cancer chemotherapy. And I will verify the reliability and the validaity of a scale for application in a clinical setting. Nineteen patients who treat with anticancer agent were enrolled in this study until March 2018. Ten patients had skin reaction by chemotherapy. There is a differnce in the purpose and meaning of self-management between adjuvand chemotherapy and the chemotherapy for recurrent or advanced cancer. So recurrent or advanced cancer patients will enroll in this study from now on. Many of nineteen female patinets performed good self-management as daily skin care. Male patients must be enrolled in this study, so I will carry out the data collection of gastrointestinal cancer patients or lung cancer patients.
