2007年から2014年に慶應義塾女子高等学校に在籍し, 同意が得られた生徒のうち, 出生時体重の申告のあった618名を対象として解析を行った。入学時の健康診断で得られた, 身体所見(身長・体重・BMI・血圧)および各種血液検査所見の解析では, 8年間の推移で, 平均血圧の上昇, 中性脂肪の上昇を認めた。また血清クレアチニンが有意に上昇し, それに伴って推定糸球体濾過量(eGFR)が低下していることが判明した。母子手帳の調査によると8年間で出生時体重は年々低下, 妊娠週数の短縮化も認めた。各年代における, 軽度腎機能障害(eGFR60mil/min未満), 低出生体重(出生時体重2500g未満)の生徒の割合は有意に増加して推移した。ロジスティック回帰分析により軽度腎機能障害となる危険因子を検討したところ, 低出生体重児は出生時体重3000g以上の生徒に比してオッズ比3.3(p=0.017)と有意な結果であった。また平均血圧, 尿酸値, 総コレステロール値も同様に軽度腎機能障害の危険因子となることが判明した。なおこれらの結果は, 妊娠週数による影響を除外するために行った妊娠週数37週以降の生徒に限った検討においても同様であった。以上より, 低出生体重は他の生活習慣病指標とともに, 高校生女子における軽度腎機能障害の危険因子となることが示唆された。同期間の高校生男子1736名についても同様の解析を行ったところ, 低出生体重児は出生時体重3000g以上の生徒に比してオッズ比1.73(p=0.031), また現在過体重である生徒は正常体重である生徒に比してオッズ比1.98(p<0.0001)であり, 低出生体重と現在の体重が軽度腎機能障害の危険因子であった。低出生体重は男女ともに若年期の軽度腎機能障害の危険因子となることが示唆された。 The aim of this study was evaluating the relationship between birth weight and renal function of healthy female Japanese adolescents aged 15-16 years who were studying at Keio Girls' High School during 2007-2014. Their mean blood pressure (BP) and serum Triglyceride (TG) level increased significantly from 2007 to 2014. There was a significant decrease in estimated glomerular filtration rate (eGFR) over time. Trends in birth weight decreased and in their gestational week became shortened across 8 years. The prevalence of mild renal dysfunction (eGFR < 60ml/min) and low birth weight (LBW ; birthweight < 2500g) increased significantly. The interactions between mild renal dysfunction and various risk factors were investigated by using multiple logistic regression analysis. Their birth weight, uric acid, total cholesterol and mean BP were the predictive risk factors for mild renal dysfunction. The odds ratio (OR) of the LBW group according to birth weight for mild renal dysfunction was high (OR, 3.30 ; p = 0.0171). These results were same in the group except for the preterm birth. We also investigated the interactions between mild renal dysfunction and various risk factors in 1736 male Japanese adolescents. Their birth weight and current body weight were the predictive risk factors for mild renal dysfunction (the OR ; LBW, 1.73, p = 0.031 ; current over weight, 1.98, p<0.0001).
