Long-term performance characterization of implantable hemodialysis system

国内で32万人, 世界では260万人が受けている人工透析治療は, 腎不全患者にとって生体腎移植の代替治療として30年以上にわたり用いられてきている。極めて完成された治療法であり, 人工透析を適切に受けることで30年以上の生存も報告されている。一方で, 患者が週3回の通院を強いられ, かつ通院毎に4時間の床上安静ならびに穿刺が必要であること。食事ならびに水分制限が厳しいことなど, 患者QOLに大きな課題がある。さらに, 透析患者一人あたり500万円以上かかるため, 2017年での医療費は1兆7000億円を越えている。そこで我々のグループでは, マイクロ流路と透析膜を組み合わせたインプラント型透析装置を開発している。本装置は, 血圧で十分な血流量を取り入れるよう設計され, また透析液を用いない血液濾過を用いることで, ポンプを不要とし, 小型化, インプラントを可能としている。患者は通院から解放されQOLが格段に向上し, また食事・水分制限も緩やかになることが期待される。さらに, 通院回数が減ることによる医療費の大幅削減に繋がる。
3年計画の1年目として, 透析装置の流路ならびに膜の表面状態と血液凝固に関する研究を行った。異なる加工方法により生成された流路の表面状態が, 血液成分の固着に影響があることを実験的に示し, 透析装置流路表面に要求される表面粗さを定量的に明らかにした。また, ラットを用いた中期実験を行うために, ラットへの適正な抗凝固剤投与量を実験的に導出した。これにより, 来年度以降はラットを用いて, 2週間程度の中期実験が可能となる。またイヌを用いた長期実験に向けて, 血管と人工血管, また人工血管同士の接続に必要なコネクタを開発した。このコネクタについては, 特許申請を行った。本年度の研究開発により, 2年目以降の中長期実験に必要な知見が蓄積され, 本研究期間(3年)の最終目標である長期動物実験に向けた準備を着実に遂行することができた。
Dialysis therapy is currently widely used for patients with end-stage renal diseases. The therapy is well-developed, however, the patients must visit hospitals three times a week and stay there for 4 h in each visit. At each treatment, patients experience puncture of thick needles. Patients must adhere to dietary restrictions. These issues lead to low quality of life (QOL) of the patients. In addition, the medical fee of the dialysis therapy is now exceeding 1.7 trillion yen. As a promising solution, we propose hybrid dialysis therapy with an implant auxiliary kidney, which is a hemofiltration system. The implant hemofiltration system removes water and other small molecules continuously and the patients receive conventional dialysis treatment once a week to balance the electrolytes and remove wastes that are not sufficiently removed by the hemofiltration. The less frequent hospital visit and alleviated dietary restriction, i.e., patients are allowed to drink water, drastically improves their QOL. It is estimated that the medical fee can be compressed to two thirds of the conventional treatment.
Our goal of this three-years project is to conduct long-term, 1 year or more, in vivo experiments to verify that the dialysis system maintains its performance for 1 year. This year, first, we investigated the effects of surfaces of the fluidic channels and membranes. Manufacturing methods affected the surface and thus the coagulation and adhesion of the blood. We quantitatively clarified the required surface quality from in vitro experiments. In order to conduct mid-term in/ex vivo experiments, we attempted to deduce the appropriate dose amount of anticoagulants. Based on this result, we presume that we can conduct 2-weeks ex vivo experiments with SD rats in the second year of this project. In addition, for long-term in vivo experiments, we developed the connectors between the blood vessels and the artificial vessels, and between the artificial vessels.
In summary, we could successfully collected sufficient data and knowledge to pursuit our final goal of this project, i.e., long-term in vivo experiments as non-clinical proof-of-concepts.
