メーカーと共同開発した世界初の立位CTを導入し、ファントム実験にて物理特性が従来の臥位CTと同等であること、臥位CTに比べ入退出時間が有意に短く、ワークフローが向上することを確認した。健常人180例で立位での人体の解剖学的構造を検討した。従来、脳は体位によって動くことはないとされてきたが、立位では位置の下垂が見られこと、肺は立位で容積が増大するが、変化率は葉により異なり、中葉は不変であること、骨盤底は女性の方が男性より、また加齢に伴い下垂の程度が大きくなることがわかった。また、静脈容積は体位により変化するが、その変化の仕方が上半身と下半身で異なり、脳内静脈は体位よる変化がないことがわかった。 We developed the world's first upright CT in collaboration with a manufacturing company, and it was introduced to our hospital for testing. Phantom studies found its physical properties to be comparable to conventional supine CT, however, with an improved workflow due to smoother entry/exit of subjects. We evaluated the anatomical structures of 180 healthy subjects in the upright position. We found that brain structures descend slightly in the upright position, despite the notion that brain position remains static regardless of body position. Also, lungs were found to have a larger volume in the upright CT compared with supine CT, however, the rate of change varied depending on the lobe, with the middle lobe being unchanged. The pelvic floor had greater descent in women than in men, and a greater descent with age. The venous volume changed depending on the position with the change varying between the upper and lower trunk and no change in the intracerebral vein.
