小児肝移植後に生じる難治性拒絶等合併症や脂肪変性等の肝組織障害と原疾患には関連性を認めた。原疾患が原因不明急性肝不全の症例では、移植後中心静脈閉塞を伴う難治性拒絶を生じる頻度が高くグラフト不全となるリスクが高かった。代謝性疾患の肝移植後にはびまん性の脂肪変性による肝障害の頻度が高いものの一過性であり、移植後拒絶を生じる頻度は低かった。肝移植後5年以上経過した症例で抗ドナー抗体陽性且つ肝生検が施行された18歳以下の小児症例において、抗ドナー抗体陽性でBanffの診断基準による慢性抗体関連拒絶の項目を満たす症例では、診断基準項目を満たさない症例と比較して線維化進行を認めた。 We found relationship between complications after pediatric liver transplantation such as rejection, liver dysfunction and original disease. Recipients of etiology-unknown acute liver failure tended to present steroid-resistant rejection which showed veno-occlusive like change, and percentage of graft failure was high, Recipients with metabolic disorders tended to present diffuse macrovesicular steatosis soon after liver transplantation, however steatosis was transient, and frequency of rejection was low. We examined liver biopsy specimens of recipients under 18-year old who had positive results of donor-specific antibody (DSA) and were followed-up over 5 years after liver transplantation. Cases which had positive-DSA and met the diagnostic criteria of chronic antibody-mediated rejection by Banff Working Group showed progression of liver fibrosis compared with cases which did not meet the diagnostic criteria.
