新たな診断機器である立位 (座位) 胸部CTが、日常臨床検査として施行可能であることを確認した。無症状ボランティアにおいて、臥位と比較して立位で、両側全肺体積・両側肺上葉体積・両側肺下葉体積は有意に大きく、さらに肺下葉においてその違いが大きいことが明らかとなった。一方で右肺中葉は、臥位と立位で体積が変わらないことが分かった。慢性閉塞性肺疾患においては、肺上葉・下葉については立位の方が臥位より体積が大きい一方、中葉については立位の方が臥位より体積が小さいことが明らかとなった。また、立位CTでの全肺体積は、臥位CTでの全肺体積と比較して、呼吸機能検査での全肺気量の値に、より近いことが明らかとなった。 We confirmed that chest upright CT can be used in routine clinical practice. In asymptomatic volunteer cohort, while the inspiratory volumes of the bilateral upper and lower lobes and bilateral lungs were significantly greater in the standing than in the supine position, with lower lobes showing larger changes, the right middle lobe volume did not change significantly between positions. In patients with chronic obstructive pulmonary disease (COPD), whereas the inspiratory volumes of the bilateral upper and lower lobes and bilateral lungs were significantly greater in the standing position than in the supine position, the right middle lobe volume was significantly lower in the standing position than in the supine position. The total lung volume on upright CT in the standing position was more similar to total lung capacity on pulmonary function test than that in the supine position in both asymptomatic volunteer and COPD cohorts.
