• Note (General): type:text; 薬物の消化管吸収過程における薬物相互作用の一つに、キレート形成による薬物吸収低下がある。なかでも、ニューキノロン系抗菌薬とアルミニウムの薬物相互作用により、抗菌薬の吸収が低下することは広く知られている。一方で、消化管吸収モデル細胞を用いた先行研究において、両剤を併用した際にアルミニウムの吸収(透過性)が増大する可能性があることを見出した。しかし、ニューキノロン系抗菌薬併用時の金属カチオンの吸収亢進が、アルミニウム以外の金属についてもみられるかは検討されていない。また、この現象が生体レベルでみられるかも評価されていない。そこで、本研究では、ラットおよび消化管吸収モデル細胞を用いて、ニューキノロン系抗菌薬併用によるマグネシウムの吸収変動について評価することを目的とした。 まず、ラットにおいて、ニューキノロン系抗菌薬の一種であるシプロフロキサシンと酸化マグネシウム製剤を併用すると、シプロフロキサシンの薬物血漿中濃度‐時間曲線下面積 (AUC) はシプロフロキサシン単独時の 20% 程度に低下した一方で、マグネシウムの AUC はマグネシウム単独時の 130% に増大する傾向が見られている。よって、酸化マグネシウム製剤とシプロフロキサシンを併用することで、マグネシウムの全身移行が亢進する可能性があり、両剤の併用には注意が必要であることが示唆された。 一方で、消化管吸収モデル細胞を用いた検討においては、マグネシウム定量法の確立が難航しており、引き続き検討を行っていく。 One of the drug-drug interactions during the drug absorption process is the decrease in drug absorption due to the chelation. For example, oral absorption of new quinolones antibiotics (NQs) is decreased by the coadministration of a drug containing polyvalent metal cations such as aluminum. On the other hand, the absorption of aluminum was increased by NQs in our previous in vitro study. The purpose of this study was to evaluate the variation of magnesium absorption by the coadministration of NQs in rats and Caco-2 cells, a model of gastrointestinal absorption. In rats, the combination of ciprofloxacin and magnesium oxide decreased the area under the drug plasma concentration-time curve (AUC) of ciprofloxacin to about 20% of that of ciprofloxacin alone. On the other hand, the AUC of magnesium tended to increase to 130% of that of magnesium alone. Therefore, it was suggested that the concomitant use of magnesium oxide and ciprofloxacin may increase the absorption of magnesium and that caution should be used in combination with both drugs.; 申請種類 : 福澤基金研究補助
