本研究はCT画像に基づく体表面データおよび骨関節位置情報から、手指変形性関節症に対するオーダーメイドの３Dプリンティング治療装具を開発し、最小限の固定範囲で効果的に罹患部位を固定することを目的とした。第1段階として健常ボランティアの3Dプリンティング装具を作成し可動範囲と固定性のバランスが適切であることを確認した。第2段階として臨床試験を開始し、本装具により全例で動作時、安静時共に疼痛の改善が得られ、また日常生活動作の改善を認めたが、長期装用での耐久性に問題点が浮上し、12.5%で装具の破損が認められた。本装具は有用であるが、臨床応用には技術的に解決すべき課題が存在する。 The purpose of this study was to develop a custom-made 3D printing orthosis for finger osteoarthritis based on CT images, and to fix the affected joint effectively. As the first step, we created a 3D printing orthosis for healthy volunteers and confirmed that the balance between range of motion and fixation was appropriate. As the second step, clinical trials were started. In all cases, this orthosis successfully improved pain during movement and at rest, and improved activities of daily living. However, there was a problem with durability after long-term wearing. Breakage of the orthosis was noted in 12.5% of cases. Although this brace is useful, there are technical challenges for clinical application.
