中耳炎は非常に一般的な疾患であり、反復すると難聴になることは知られているが、その難聴になる機序は未解明である。この機序が解明されれば難聴の予防、治療が可能となりうるという点で非常に重要である。仮説としてはIL-1やIL-6などのサイトカインに誘導されて破骨細胞が増加することにより、耳小骨が溶解、固着するため、もしくは感染により酸が発生し直接的に骨が溶解されるためというものがある。そこで、今回我々は破骨細胞の増加によって耳小骨の溶解、固着が生じるという仮説を検証した。右耳にLPS(エンドトキシン)投与された中耳炎モデルマウスを作成し、左耳にはコントロールとして生理食塩水を投与した耳と比較した。両耳の耳小骨の破骨細胞をTartrate-Resistant Acid Phosphatase(TRAP）染色にて比較した。その結果、コントロールに比べ中耳炎を起こした耳の耳小骨(ツチ骨、キヌタ骨、アブミ骨）では破骨細胞の数が著しく増えていることが観察された。この結果は、破骨細胞の増加により骨破壊が生ずることを示しており、耳小骨の変形が反復感染によって生じることで難聴をきたすことが考えられる。また、pH測定にでは酸性になっておらず、この反応はLPS投与によるものであり、酸によって生じていないことを確認した。 Otitis media is a commonly occurring condition of the middle ear. Recurrent otitis media is known to cause hearing loss, although the mechanism of otitis media-related hearing loss remains to be clarified. It is important to elucidate this mechanism to prevent or effectively treat otitis media-related hearing loss. According to existing hypotheses, hearing loss occurs because an increase in the quantity of osteoclasts induced by cytokines leads to ossicular lysis or fixation or because acids increased in quantity as a result of infection directly cause ossicular lysis. To test the first hypothesis, otitis media was induced in mice by injecting a lipopolysaccharide (LPS; endotoxin) into the right ear. Left ears in which saline was injected acted as controls. On comparing the control and diseased tartrate-resistant acid phosphatase-stained ear osteoclasts, there seemed to be a markedly higher number of osteoclasts in the study ear than in the control ear. We measured it was not acidic for pH measurement, and this mechanism was due to LPS administration and was not caused by acid.
