• Note (General): type:text; う蝕や歯周病は糖尿病や心疾患、誤嚥性肺炎等、全身の疾患の原因にもなり得るため、口腔内環境の維持は重要である。しかしながら、我が国の過去1年間の歯科検診受診率は53%に留まっている（2016年）。一方で、薬局には国民の健康の維持・増進を支援する機能が求められている。2015年には薬局店頭での唾液による口腔内環境チェックが可能となったが、薬局にて唾液検査の有用性を検討した報告はまだない。そこで本研究では、薬局来局患者を対象に唾液検査及び口腔セルフケアに関する情報提供を行うことで、薬局での口腔内環境チェックのニーズ及び口腔セルフケア習慣や歯科受診行動に与える影響を明らかにすることを目的とした。2018年6月11日～2018年7月13日に慶應義塾大学薬学部附属薬局の来局者84名（男性38名、女性46名）を対象に、唾液検査及び口腔セルフケアに関する情報提供を実施し、合わせて自記式質問紙調査を行った。唾液検査について「結果がその場で分かる」(81%)、「測定が簡単」(70%)、「口腔セルフケアを見直すきっかけになる」(60%)の回答が多く挙がり、患者が検査の簡便性や有用性を実感したことが示唆された。唾液検査及び情報提供の満足度は96%に上り、89%が薬局で唾液検査をまた受けたいと回答したことから、高いニーズが示された。1か月後追跡調査（回収率：67%）により、唾液検査及び情報提供の実施前後で「甘い食品や飲料を控えている」割合が43%から61%へ有意に増加した(P=0.021)。また、定期歯科検診を受けていなかった12名中1名(8%)が新たに歯科を受診し、6名(50%)が「受診しようと思っている」と回答したことから、唾液検査や情報提供が歯科受診に対する意識の変化を促した可能性が示唆された。4か月後の時点では、さらに1名が新たに歯科を受診していた。本研究により、薬局における口腔内環境チェックが地域住民の口腔疾患の早期発見及び予防に有用であることが示唆された。 It is important to maintain the oral environment since caries and periodontitis are said to affect systemic diseases, such as diabetes mellitus, cardiovascular disease, and aspiration pneumonia. However, only 53% of Japanese people undergo a regular dental checkup. Community pharmacies are required to support the health promotion and maintenance of local residents. In 2015, saliva test was allowed to be performed at Japanese community pharmacies. In this study, we aimed to reveal the needs and usefulness of the saliva test and providing information about oral self-care at community pharmacies on the oral self-care habits and dental consultation. We carried out the saliva test, providing information, and a questionnaire survey to 84 patients (38 men and 46 women) at University Community Pharmacy, Keio University, from June 11, 2018 to July 6, 2018. 96% of the patients were satisfied with the saliva test and providing information. The patients mainly cited "available results of the test on the spot" (81%), "easiness of the test" (70%), "motivation to review my oral self-care" (60%) as an impression against the saliva test. Additionally, 89% of the patients desired to take the saliva test at community pharmacies again in the future. Therefore, the saliva test at community pharmacies is confirmed to be needed by local residents. After one month, the follow up questionnaires were returned from 67% of the patients. The rate of patients who refrained from sweet foods and beverages significantly increased from 43% to 61% (P = 0.021). 1 of 12 patients who didn't undergo a regular dental checkup newly visited dental office, and 6 had the intention to do so. Thus, it was speculated that the saliva test and providing information possibly enhance their consciousness to dental consultation. Additional 3 months later, another 1 patients newly visited dental office. In conclusion, it was indicated that oral checkup at community pharmacies is useful for the early detection and prevention of oral diseases among local residents.
