• Note (General): type:text; 本邦で大腸がん（colorectal cancer; CRC）の発生、死亡が増加しており、CRCの発生要因として生活習慣、メタボリック因子との関連が示唆されているが、本邦での両者の関連を示す大規模な報告はない。今回、CRCの前癌病変と考えられている大腸腺腫性ポリープの発生に対するリスク因子について、人間ドックにおける総合的な健診データを用いて、メタボリック症候群（MetS）に焦点をあてて検討を行った。 2012年8月から2015年7月の間に当院で総合的な人間ドック健診を行った7213名の受診者中、大腸内視鏡検査を受けた者が1835名で、胸部CTを受検しなかった者を除外した1772名を対象とした。年齢、性別、身体計測（体格指数（BMI））、CTにより計測した内臓脂肪面積（VFA）、血圧、血液生化学的データ（脂質代謝、糖代謝）、高血圧、脂質異常症、糖尿病の治療歴を独立変数として検討を行った。大腸腺腫性ポリープ（腺癌を含む）と各変数との関連を単変量解析により評価し、さらに多重ロジスティック回帰モデルを用いた多変量解析により検討を行った。 対象のうち506例（28.6％）に大腸ポリープを認め、進行がんは5例（0.28％）であった。446例（25.2％）がMetsに該当していた（男性31.3％、女性12.0％）。単変量解析では性別、年齢、BMI、VFAがポリープと有意な関連性を認め、また、MetSは有意に大腸ポリープと関連していた（P<0.01）。ロジスティック回帰分析では、性別と年齢、内臓脂肪蓄積、さらにMetSを構成する要因として糖質異常がそれぞれ大腸ポリープの独立したリスク因子であった。 今回の研究により加齢、肥満、男性およびメタボリック症候群に該当することが日本における大腸腫瘍発生のリスク因子である可能性が明らかとなった。今後、アディポネクチン等のアディポサイトカインの測定データを統合し、大腸癌発生を予測するアルゴリズムの開発を行う。 For the past decades, the incidence and mortality rate of colorectal cancer (CRC) has been increased in Japan. The association of metabolic syndrome (MetS) and CRC has been reported in several studies, however, individual factors contributing to development of CRC have been obscure, especially in Japan. In the present study, we investigated the risk factors, especially focusing on MetS for development of colorectal adenomatous polyps (CP) as precancerous lesions by using comprehensive health checkup data. Among 7213 subjects who took comprehensive health checkup at our hospital between in August 2012 and July 2015, 1772 subjects who also underwent screening colonoscopy were analyzed. Demographic characteristics, anthropometric measurements, visceral fat area (VFA), blood pressure and hematological metabolic parameters were assessed. Association between variables and CP was evaluated by univariate analysis, and then by multivariate analysis using multiple logistic regression model. A p value < 0.05 was considered statistically significant. Of 1772 subjects analyzed, 506 subjects had CP (28.6%) and 5 had invasive colorectal cancer (0.28%). Four hundred and forty-six subjects were diagnosed as MetS (25.2%) and presence of MetS was significantly associated with CP (p < 0.01). Univariate analysis identified significant association of sex, age, body mass index (BMI), and VFA with prevalence of CP. Logistic regression analysis revealed that sex, age, VFA and abnormal blood glucose, which is one of factors consisting of MetS, were independent factors associated with CP. The present study demonstrated that male, aging, obesity and MetS might be risk factors for colorectal neoplasia in Japan.
