• Note (General): type:text; 【目的】同種造血幹細胞移植 (HSCT) 後のhuman herpesvirus-6 (HHV-6) 再活性化は重篤な中枢神経障害を引き起こす。近年、HHV-6特異的受容体であるCD134の移植後CD4陽性細胞での発現がHHV-6再活性化に関与する可能性が報告され、注目されている。本研究では、高感度にウイルスDNA量を測定可能なデジタルPCR (dPCR) を用いて、CD4陽性T細胞におけるCD134陽性率とHHV6再活性化の関連性について検討した。【対象と方法】当院で同種HSCTを施行した患者34例（臍帯血移植13例、HLA不適合血縁/非血縁者間移植1例/5例、HLA適合移植15例）を対象に、前処置前より移植後4週まで毎週、血漿中のHHV-6 DNA量をdPCRで測定した。同時にフローサイトメトリーでCD4+ T細胞におけるCD134陽性率 (CD134/CD4比) を評価した。【結果】dPCRでは従来のリアルタイムPCR（HHV-6 DNA検出下限 200コピー/mL）と比較してHHV-6 DNAの検出感度の向上が得られ、HHV-6 DNA 7コピー/mL以上をHHV-6 DNA陽性（＝再活性化）と定義した。HHV-6再活性化はdPCRで23例 (68％)に認められた。多変量解析では、前処置前のCD134/CD4比高値 (オッズ比 (OR) ＝ 2.4, 95％信頼区間 (CI) : 1.1– 5.3, <P ＝ 0.02) と、臍帯血移植を含むHLA不適合移植がHLA適合移植と比較して (OR ＝ 16.0, 95％CI : 1.7– 149.0, P ＝ 0.02) HHV-6再活性化の高リスクとなることが抽出された。一方、移植後のCD134/CD4比はHHV-6再活性化に影響を与えなかった。【考察】移植前のCD134/CD4比高値がHHV-6再活性化のリスク因子となることが明らかとなり、HSCT前にCD134陽性CD4陽性T細胞を測定することでHHV-6再活性化の高リスク症例が抽出できる可能性が示唆された。 Background: Human herpesvirus 6B (HHV-6B) causes life-threatening central nervous system disorders such as encephalitis after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Recent studies implicated CD134 as a specific receptor of HHV-6B and demonstrated that its expression levels in CD4-positive T cells after allo-geneic HSCT could be related to the reactivation of HHV-6B. We prospectively evaluated the relationship between HHV-6B reactivation and CD134+ T cells in the recipients of allo-geneic HSCT. Methods: HHV-6B viral load in plasma was quantitatively measured weekly after allo-geneic HSCT by digital polymerase chain reaction (dPCR) in 34 patients. The ratio of CD134 in CD4+ T cells (CD134/CD4 ratio) was serially measured by flow cytometry before and after transplantation. Results: HHV-6B reactivation was detected in 23 patients (68%) with dPCR. The CD134/CD4 ratio before conditioning was significantly higher in patients with HHV-6B reactivation than in those without (median [range], 3.8% [0.4–25.6] vs. 1.5% [0.4–3.7], P < 0.01), whereas the CD134/CD4 ratio was not associated with HHV-6 reactivation at any point after transplantation. In multivariate analysis, a higher CD134/CD4 ratio before conditioning was significantly associated with the incidence of HHV-6B reactivation (odds ratio = 10.5, 95% confidence interval: 1.3–85.1; P = 0.03). Conclusions: A higher CD134 expression rate in /CD4 ratio+ T cells before conditioning was associated with a higher risk of HHV-6B reactivation, suggesting that the rate may be a promising marker for predicting HHV-6B reactivation after allogeneic allo-HSCT.
