癌免疫療法は、外科療法、化学療法などに続く革新的な新規治療法として注目されている。その中でも抗PD-1/PD-L1療法は悪性黒色腫や肺癌治療に対して良好な臨床効果が認められ、今後も種々の悪性腫瘍に対する非常に有望な治療薬として期待されているものの、治療対象患者選定における重要な指標である癌細胞PD-L1の発現調節機構については不明な点も多い。本研究では、免疫組織化学的手法を用いて、ヒト肺癌組織での癌細胞PD-L1発現レベルと腫瘍内免疫細胞プロファイルを比較・検討することにより、癌細胞PD-L1発現誘導と腫瘍間質免疫環境との関連を検討した。PD-L1発現陽性例は早期肺腺癌146例中27例（18.5%）で、全例浸潤癌であった。これらの症例では、腫瘍内においてCD163あるいはCD204陽性腫瘍関連マクロファージの有意な浸潤数増加が認められ、その局在はPD-L1陽性癌細胞の局在と一致する傾向がみられた。また、ヒト由来マクロファージとの共培養実験系においてヒト肺腺癌細胞株におけるPD-L1発現増加が認められた。以上の結果から、腫瘍関連マクロファージの浸潤は癌細胞PD-L1発現誘導に直接関与している可能性が示唆され、免疫チェックポイント分子に加え、その発現誘導に関わる腫瘍間質細胞は癌免疫療法の新たな治療標的になりうる可能性が考えられた。 Programmed death-ligand 1 (PD-L1) is an immune modulator that promotes immunosuppression by binding to PD-1 of T-lymphocytes. In this study, we evaluated the association of tumor PD-L1 expression and immune cell infiltrating patterns in 146 cases of early lung adenocarcinoma. By immunohistochemistry, cell surface PD-L1 expression in tumor cells was observed in 18.5 % of Stage IA lung adenocarcinoma patients. Tumor PD-L1 positivity was significantly associated with stromal invasion accompanied by increased various immune cells, and TAMs besides CD8+ T cells were significantly accumulated in PD-L1-positive carcinoma cell areas showing a tumor-infiltrating pattern. Our in vitro experiments revealed that tumor PD-L1 expression was significantly upregulated by co-culture with M2-differentiated macrophages. These results indicate that the tumor-infiltrating TAMs are one of extrinsic regulators of tumor PD-L1 expression and targeting TAMs might be a potential strategy to enhance the response to anti-PD-1/PD-L1 therapy.
