• Note (General): type:text; 【目的】現在のシクロスポリン（CyA）経口製剤は従来の製品よりも吸収における胆汁の影響が少ないとされているが、肝移植症例において胆汁が外瘻化されている期間でCyA服用後2時間の血中濃度（C2）が低い場合を経験している。そこで今回、CyA投与症例において、経口CyAのC2と胆汁量との関係ならびに静注CyA投与開始後4時間の血中濃度（C4）と胆汁量との関係を解析した。 【対象と方法】2008年4月～2013年6月の期間に慶應義塾大学病院において生体肝移植術が実施され、経口CyAおよび静注CyAが投与された患者16例を対象とした。CyAの血中濃度や関連の臨床検査値は電子カルテを用いレトロスペクティブに調査した。 【結果】ネオーラルⓇの体重あたりの1回投与量、CyAC2および1日の外瘻胆汁量は、それぞれ1.4 ± 0.4 mg/kg/回、483.8 ± 210.8 ng/mL、187.6 ± 152.6 mL（平均値 ± 標準偏差）であった。外瘻胆汁量が多いほどCyAのC2/D比が低い結果となった（p ＜ 0.01）。一方、静注製剤CyAの投与量、CyAC4および1日の胆汁の外瘻量は、それぞれ0.9 ± 0.6 mg/kg/回、684.5 ± 201.8 ng/mL、186.9 ± 162.7 mL（平均値 ± 標準偏差）であったが、胆汁の外瘻量とCyAのC4/D比には相関関係は見られなかった。経口製剤において1日の外瘻胆汁量を100 mL以下の群と100 mLより多い群に分けた時のCyAのC2/D比の平均値はそれぞれ483.3 ± 157.1、289.8 ± 106.1 kg/L（平均値 ± 標準偏差）であり、100 mL以下の群で有意に高い結果となった（p ＜ 0.01）。 【考察】以上の結果より、CyA静注投与時のC4は胆汁量の影響をあまり受けないが、CyA経口投与時のC2は、胆汁量の影響を受ける可能性が示唆された。CyA経口投与時には、胆汁が外瘻化されている期間においてCyAの体内動態が不安定になる可能性がある事から、より頻回のCyAC2モニタリングを行い、用量を設定する事が重要と考えられる。 Purpose: Bile is considered to have less effect on the absorption of currently used oral preparations of cyclosporine (CyA) than on older preparations, but we have encountered some patients who had low blood concentration of CyA at 2 h after administration (C2) during external biliary drainage following liver transplantation. Therefore, in this study, we analyzed the relationship between C2 of oral CyA and bile volume, as well as the relationship between CyA blood concentration at 4 h after intravenous administration (C4) and bile volume. Subjects and Methods: The subjects were 16 patients who underwent living donor liver transplantation at Keio University Hospital between April 2008 and June 2013 and received oral and intravenous CyA. The blood concentration of CyA and related laboratory values were determined retrospectively from medical records. Results: The body weight dose of Neoral®, CyA C2, and daily external biliary drainage volume were 1.4 ± 0.4 mg/kg/dose, 483.8 ± 210.8 ng/mL, and 187.6 ± 152.6 mL (mean ± SD). Higher external biliary drainage volume was associated with lower C2/D (C2 divided by dose) of CyA (p < 0.01). The dose of intravenous CyA, CyA C4, and daily external biliary drainage volume were 0.9 ± 0.6 mg/kg/dose, 684.5 ± 201.8 ng/mL, and 186.9 ± 162.7 mL (mean ± SD), and there was no correlation between external biliary drainage volume and C4/D (C4 divided by dose) of CyA. On dividing patients administered the oral preparation into a low bile volume group (≤ 100 mL/day) and a high bile volume group (> 100 mL/day), the mean C2/D of CyA was significantly higher in the low bile volume group at 483.3 ± 157.1 versus 289.8 ± 106.1 kg/L (mean ± SD) (p < 0.01). Discussion: Bile volume has minimal effect on C4 of intravenously administered CyA, but may affect C2 of orally administered CyA. Because oral administration of CyA may cause pharmacokinetic instability of CyA during external biliary drainage, it is important to monitor CyA C2 more frequently and select the appropriate dose.
