• Note (General): type:text; 原虫感染症診断においては、遺伝子増幅法が有用である。しかしながら、原虫感染症流行地域においては、高価な機器や、試薬の保存・輸送に冷凍庫が必要なPCR法は一般的な検査法として実施し得ない。そのため、特別な機器を必要としない等温遺伝子増幅法であるLAMP (loop-mediated isothermal amplification) 法が、発展途上国で実際に応用されている。 本年度の研究で我々は、上記のLAMP法に加え、等温遺伝子診断法であるRPA (recombinase polymerase amplification）法と、遺伝子増幅反応物を検出紙で検出するDNA-chromatographyを組み合わせた、原虫感染症診断法の開発を行った。 本年度は、Entamoeba histolytica、Toxoplasma gondii、Giardia interstinalisをターゲットに、RPA＋DNA chromatography診断系の確立を行った。結果としては、E. histolyticaについては、0.1 parasites/μLまでの検出が可能であり、これは既存のPCR法と同等の検出感度であり、またLAMP法よりも高い検出感度を示した。G. interatinalisについては、Assemblage Aで0.1 parasites/μL、Assemblage Bで0.1 parasites/μLまで検出が可能であった。Toxoplasma gondiiについては、RPA法までの検討を行い、0.01 parasites/μLで検出可能であった。 今後は臨床検体を用いて感度・特異度を明らかにしていく予定である。 Gene amplification is useful in the diagnosis of protozoal infections. PCR method needs the expensive equipment and a freezer for storage and transport of reagents, therefore PCR can not be used generally in the protozoal infection epidemic area, Loop-mediated isothermal amplification (LAMP) method, which is an isothermal gene amplification method that does not require special equipment, is actually applied in developing countries. We tried to develop LAMP method and RPA (recombinase polymerase amplification) method, which is an isothermal gene diagnosis method as gene amplification methods, combined with DNA-chromatography that detects gene amplification reaction products with lateral flow paper. We established RPA + DNA chromatography diagnostic system targeting Entamoeba histolytica, Toxoplasma gondii and Giardia interstinalis. As a result, for E. histolytica, detection up to 0.1 parasites /μL is possible, which is equivalent to the detection sensitivity of the existing PCR method and shows higher detection sensitivity than the LAMP method. As for G. interatinalis, the detection limit is 0.1 parasites / μL with Assemblage A and 0.1 parasites/μL with Assemblage B. With regard to Toxoplasma gondii, the study was conducted up to the RPA method, and the detection limit is 0.01 parasites μL. We will clarify the sensitivity and the specificity of these tests using clinical specimens in the future study.
