• Note (General): type:text; 2007年から2014年に慶應義塾高等学校、同志木高校、同女子高等学校、同湘南藤沢高等部に在籍し、同意が得られた2,417名（男子1,736名、女子681名）を対象とし、母子手帳より聴取した出生時体重、在胎週数および高校在籍中の定期健康診断結果をもとに、生活習慣病発症における危険因子について解析を行った。出生時体重は8年間の推移にて男女ともに有意な低下を認め、女子では在胎週数が短縮していた。定期健康診断結果の解析では男女ともに推定糸球体濾過量（eGFR）が低下しており、各年代における軽度腎機能障害（eGFR 60ml/min未満）の生徒の割合は男女ともに有意に増加した。また女子では収縮期および拡張期ともに血圧の上昇を認めた。出生時体重別にみると、男女ともに出生時体重が低い群ほど軽度腎機能障害の生徒の頻度が有意に高値であった。低出生体重児（出生時体重2500g未満；LBW）は出生時体重3000g以上の生徒に比して軽度腎機能障害発症危険因子としてのオッズ比が男女ともに高値を示したが、高血圧症発症に対しては有意な所見を認めなかった。また多変量解析にて男女ともに出生時体重はeGFRの独立した予測因子であり、男子の方がより影響を受けることが判明した。エストロゲンが腎機能に保護的に働くことが報告されていること、また本研究において男子では女子に比較して過体重の頻度が高く、男子においてのみ現在の過体重が出生時体重と並び軽度腎機能障害の危険因子に挙げられることが男女差を生じる原因となっていると推測される。以上よりLBWは、明らかな生活習慣病を発症していない若年世代の男女において軽度腎機能障害発症の危険因子となり、将来を見据えた生徒の健康教育をしていくうえで、出生時体重は重要な情報になると考えられた。 The aim of this study was evaluating the relationship between birth weight and life-style related disease. The study participants included 2,417 healthy Japanese adolescents (1,736 males and 681 females) who were studying at Keio Senior High School, Keio Shiki Senior High School, Keio Shonan Fujisawa Senior High School and Keio Girls Senior High School during 2007-2014. In both male and female adolescents, their birth weight and estimated glomerular filtration rates (eGFR) decreased significantly across the 8-year study period. The prevalence of mildly reduced renal function (eGFR<60ml/min/1.73m2) increased. Only in female, blood pressure increased between 2007 and 2014. According to birth weight categories, the prevalence of mildly reduced renal function was more common in the lower birth weight group than in the normal birth weight group in both sexes. We investigated that the interactions between mildly reduced renal function and various risk factors using multiple logistic analysis. The odds ratios for mildly reduced renal function were higher in the low birth weight (birth weight < 2500g; LBW) groups in both sexes. Multivariate analysis also showed that birth weight was an independent predictor of eGFR in both sexes and that males were more affected. It has been reported that estrogen works protectively for renal function. In this study, the frequency of current overweight was higher in male compared with female, and only in male, current overweight was also one of risk factors of mildly reduced renal function. LBW is a risk factor for the development of mildly reduced renal function in healthy Japanese adolescents before overt manifestations of lifestyle-related diseases, and birth weight is useful information for health education.
