• Note (General): type:text; 特定機能病院では未承認医薬品、承認済み医薬品の禁忌・適応外使用(適応外等)を把握し、その妥当性を評価することが義務付けられている。しかし多忙な医療スタッフに全ての適応外等事例を報告させるのは多大な負担である。電子カルテからの機械的な抽出を考えた場合、適応外等の実例はデータベース化されておらず、電子カルテ上の患者の病名とデータベースとを照合して適応外等を検出する事は不可能である。本研究では適応外等データベースを人工知能 (AI) を用いて構築すること、これを利用して適応外等をモニタリングするシステムを構築することを目的とした。このために①AI を利用して文献やガイドラインから特定の薬剤と紐付けられる病名・症状名を抽出し、適応外等のリストを作成する。文献情報から適応外等の有効性・安全性に関わる情報を併せて抽出し、情報源のエビデンスレベルに基づいて投薬の妥当性を提示させる。②電子カルテから患者の年齢、性別、アレルギー・副作用歴、臨床検査値、病名、妊娠の有無、投与薬剤名等を抽出し、適応外等データベースと突き合わせて適応外等を検出する仕組みを構築することとした。本研究は3年の研究期間を予定しており、2019 年度はその2年目である。 ①についてはプロトタイプを作成した。「医療上の必要性の高い未承認薬・適応外薬検討会議」で公表されている医薬品をサンプルとし、それらと病名・症状名を紐付けた文献等の抽出を行い、13医薬品中、8医薬品で文献の抽出が可能であった。 ②についてもシステムを構築し、4種類の薬剤を対象に実際の電子カルテの情報から禁忌の投与事例を抽出することができた。 3年目は、①、②について検出の真度・精度を向上させる。 Special functioning hospitals are obliged to take control of the use of unapproved drugs, contraindications and off-label use of approved drugs, and evaluate their relevance. However, it is a heavy burden for busy medical staff to report all cases. Furthermore, since off-label use is not stored in a database, it is impossible to detect them from electronic medical records. The purpose of this study was to construct a off-label use database using artificial intelligence (AI), and to construct a monitoring system using this database. To achieve this (1) Extract diseases / symptoms associated with a specific drug from the literature and guidelines using AI and create a list of off-label uses. It also extracts information related to efficacy and safety of the of off-label use from literature, and provide validity based on the evidence level. (2) Extract the patient's age, gender, allergy, medical and side effect history, laboratory test, pregnancy status, administration drug, etc. from the electronic medical record, and construct a mechanism to detect off-label use. A prototype for (1) had been built. Drugs published in "The Evaluation Committee on Unapproved or Off-labeled Drugs with High Medical Needs" were used as samples, and literatures linked to them by disease / symptom were extracted. Literature extraction was possible for 8 drugs out of 13. The system for (2) was also built. Contraindication cases could be extracted from electronic medical record for four types of drugs. In the next year, the accuracy and the precision of the detection will be improved.
