• Note (General): type:text; 今年度の活動として文献検討および研究計画の検討を行い、以下の３つの成果を得た。 1点目は、基準関連妥当性を検証する尺度の選定である。助成金申請段階では、基準関連妥当性を検証する尺度は今後検討としていた。文献検討により、Abbey pain scale（6項目）、PAINAD（5項目）、DOLOPLUS-２（10項目）を用いることが有用である可能性が示唆された。これらが有用である理由は、日本語版の信頼性・妥当性が検証されていること、項目数が少なく、回答する医療・介護スタッフの負担が少ないという点である。 2点目は、研究対象となる認知症高齢者の選定基準を検討した。認知症高齢者がエンド・オブ・ライフ期にあることを研究において判断するには、英国のGold Standards Framework (GFS)や米国のホスピス導入期基準が主に用いられている。しかし、この指標を用いた場合、標準化された尺度による認知機能の評価や血液検査による血清アルブミン値の確認などが必要であることから、十分な医療設備や、認知症の専門知識を有する人材を持たない介護施設では、この基準を用いた調査は困難である。そこで検討過程では認知症発症後の予後が10年程度であり認知症の診断があれば数年後に死が予測されることに着目すると、診断のついている認知症の人は、手厚いケアの対象となるべきエンド・オブ・ライフ期と判断できると考えた。上記より、介護施設を主なフィールドとする本研究では、研究対象となる基準を「医師から認知症と診断されている者」とする予定とした。 3点目に文献検討により本研究の意義が明確となった。先行研究では、エンド・オブ・ライフ期にある認知症高齢者は、適切な症状アセスメントが受けられず、症状緩和が行われていないことが報告されている。本研究による認知症高齢者の症状アセスメントを可能とする尺度の開発は、高齢者の苦痛が少ない穏やかな死の実現に貢献する可能性があることが示された。 We accomplished following three points from a literature review and considering a study protocol. we clarified the following three points. First, some scales were selected as potential criteria to assess the criterion-related validity. When we applied this grant, we did not have no idea for the potential measurements for criterion validity assessment. The literature review conducted in 2019 suggested that it might be useful to use Abbey pain scale (6 items) PAINAD (5 items), and DOLOPLUS-2 (10 items). The reasons for the usefulness includes verified reliability and validity of the Japanese version for these scales and low burden for survey participants (healthcare / nursing staff) to response owing to small numbers of items. Second, we examined the inclusion criteria to select older people with dementia as participants of our research. When researchers need to judge if older people with dementia are in end-of-life or not, the judgement usually have been mainly based on the Gold Standards Framework (GFS) in the United Kingdom and the hospice introductory standard in the United States. However, these criteria need evaluation of cognitive function using a standardized measurement and blood tests for serum albumin levels; these framework and standard are not useful for our research conducted in long-term care facilities because of limited medical equipment and human resources. Therefore, we considered that people with diagnosed dementia can be regarded in end-of-life periods to be cared with special attention considering their anticipated close death (a few years) owing to short average life expectancy of persons with dementia which is supposed to be about ten of years after their diagnosis. We decided participants inclusion criterion as "Persons who are diagnosed as dementia as physicians" for our research which will be conducted in long-term care facilities. Third, the significance of our research was clarified by literature review. Previous studies revealed that older people with dementia in their end-of-life often could not receive appropriate symptom assessment and palliation. These studies indicated that developing the scale for symptom management of demented older people in our research has potential to contribute quality and peaceful death withor pain for older people with dementia.
