• Note (General): type:text; PSCは高率に炎症性腸疾患を合併し、抗生物質の投与により肝胆道系酵素の改善が認められることから、PSCに特異的な腸内細菌が存在し、免疫細胞との相互作用を介して病態に寄与する、と仮説を立て、新規治療法の開発による臨床への還元を目的として2015年に研究に着手した。患者由来腸内細菌を無菌マウスへ移植したヒトフローラ化マウスを用いた検討を行い、PSC患者由来糞便微生物を移植したヒトフローラ化マウスの腸間膜リンパ節に"侵入"したbacterial translocation菌を分離培養することに成功した。本手法を用いて、PSCマウスの腸間膜リンパ節から Klebsiella pneumoniae (KP)を分離同定し、PSC患者において健常人、炎症性腸疾患患者と比較して本菌が高率に検出されることを明らかにした。Bacterial translocationの結果、肝臓内に誘導されたIL-17産生性T細胞 (TH17)がPSC様胆管障害・肝線維化の増悪に直接関与することから、上記腸内細菌が腸肝循環を介して肝病態の進展に寄与することが示唆された。さらに、ヒト大腸上皮2次元オルガノイドと腸内細菌の培養系を用いた検討により、PSC患者由来KPは大腸上皮にアポトーシスを介して障害(pore formation)を誘導することを見出した。 東京医科歯科大学の芦田先生との共同研究のもと、pore-forming Kp株 9株全てが有し、pore formation能がないKp株 5株が有さない2遺伝子(KatE, HigA)を欠損させた遺伝子破壊Kp株を作製し、遺伝子の欠損をPCRで確認した。両株を用いてpore formation能、およびマウスにおけるbacteral translocationを検討したところ、予想に反して大腸上皮障害能の軽減は認められず、両遺伝子のpore formation能への直接的関与は否定的であった。現在、海外のPSC患者から分離された約30のKp株を用いて各株のpore formation能を検討しており、今後より多くの遺伝子情報に基づいた上皮障害の分子メカニズムの解明を進める予定である。 Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease characterized by the development of bile-duct strictures and the destruction of biliary trees that lead to end-stage liver cirrhosis. PSC is characterized by the frequent complication of inflammatory bowel diseases and antibiotic treatment has improved the elevated serum hepatobiliary enzymes, suggesting the role of gut microbiota on the pathogenesis. The applicant has established humanized microbiota mice (HMA) transplanted with fecal microbiota derived from PSC patients and analyzed the immune responses in extra intestinal organs including liver, which enable us to identify the specific bacterial species directly contributing the pathogenesis. The composition of gut microbiota of PSC patients demonstrated a distinctive characteristic with a higher prevalence of Klebsiella pneumoniae, an intra-oral indigenous bacteria. Further analysis revealed that Klebsiella pneumoniae from PSC patients has a potential to induce an intestinal epithelial damage with the strain-specific manner, leading to bacterial translocation and TH17 immune responses in the liver. We have demonstrated that 2 genes (KatE, HigA) were present in 9 out of 9 pore-forming Kp strains, while absent in all non-pore forming Kp strains from the whole genome analysis. Therefore, we produced KatE or HigA gene-disrupted pore-forming KP strains (ΔKatE KP and ΔHigA KP), and deletion of each gene was confirmed by PCR. However, both constructed KP strains retained the epithelial pore-forming ability both in vitro and in vivo, suggesting that these genes may not directly contribute to this function. We are planning to construct other gene-disrupted KP strains to clarify the genomic mechanism of epithelial pore formation. This point is critical particularly in view of a future therapeutic potential.
