• Note (General): type:text; PED5阻害薬であるTadalafilは、実臨床にて前立腺肥大症による排尿障害改善薬、勃起不全改善薬、肺高血圧治療薬として使用されている。現在、筋層非浸潤性膀胱癌においてスタンダードな抗癌剤膀胱内注入療法とTadalafilを組み合わせることにより、より強力な抗腫瘍効果を発揮し実臨床へと橋渡しすることを目的として本研究を遂行している。 (1)-In vitroでのTadalafilの抗腫瘍効果の確認- 膀胱癌細胞株3種(T24、5637、UMUC-3)とマウス膀胱癌細胞株1種(MBT2)を用い、通常臨床にてTadalafil投与時の血中濃度とほぼ同量(100μg/mL)を用いて実験を行った。Tadalafil単剤では抗腫瘍効果を認めなかったものの、Mitomycin C（1μg/mL)に付加することにより、すべての細胞株においてその相乗効果をWST assayにて確認した。 (2)-酸化ストレスに着目した機能解析- 上記実験で特に優位に抗腫瘍効果を認めたT24細胞株を用い以下機能解析を行った。Mitomycin CとTadalafil 併用群において特にPDE５活性の低下を認め、それに伴いcGMP濃度の上昇を確認した。また併用群においてSOD活性、カタラーゼ濃度、MDA濃度および酸化型/還元型グルタチオン比が大きく変動しており、酸化ストレスが抗腫瘍効果に関与している可能性が示唆された。 (3)-In vivoでのTadalafilの抗腫瘍効果の確認- 細胞株MBT2を用いてマウス膀胱癌同所性モデルに対する、Mitomycin C(４日に１回50μg膀注)とTadalafil(毎日20μg腹腔内投与)との併用療法をin vivoに検討した。Day30に膀胱を摘出し腫瘍サイズを確認した。Mitomycin CとTadalafilとを併用することにより、膀胱腫瘍サイズの低下を認めた。摘出膀胱の病理組織にて、Mitomycin C投与単独群では膀胱筋層肥厚および線維化炎症細胞の浸潤傾向を認めたものの、併用群においてその抑制効果を認めた。 PDE5 inhibitor Tadalafil is the medicine for urinary difficulty with prostatic hyperplasia, erectile dysfunction, and pulmonary hypertension in clinical practice. The aim of this study is to strengthen the antitumor effect against non muscle invasive bladder cancer by combining the standard intravesical chemotherapy and Tadalafil. (1)-In vitro analysis- We performed WST assay to examine the antitumor effect in three bladder cancer cell lines (T24, 5637, UMUC-3), and one mouse cell line (MBT-2), combining Tadalafil (100μg/mL, normal serum density in clinical use) and Mitomycin C（1μg/mL). Then we confirmed the synergistic effect in all four cell lines. (2)-Functional analysis focusing on oxidative stress- For the functional analysis of Tadalafil, we examined the oxidative stress markers. We confirmed that PDE5 activity is suppressed, and cGMP density increased by the combination treatment. Furthermore, oxidative stress markers (SOD activity, catalase, MDA, and ratio of oxidized/reduced glutathione) dramatically changed by the combination treatment. (3)-In vivo analysis- We examined the antitumor effect in vivo, administering 50μg Mitomycin C intravesically and 20μg Tadalafil intraperitoneally to the orthotopic bladder cancer model. Then, we confirmed the reduction of tumor size in the combination group. Pathologically, we found the Tadalafil could prevent the inflammatory change of bladder muscle layer which was induced by the Mitomycin C.
