• Note (General): type:text; 難治がんの代表である膵癌に対する治療が模索される中、腫瘍間質の修飾により免疫治療効果が顕著に増強されることが明らかとなりつつある。本研究は、膵癌に対する免疫間質療法の実用化・個別化への展開の基盤となる成果を創出する。具体的には、患者組織の精緻な定量解析を用いて、ヒト膵癌においてT細胞性免疫の抑制に中心的役割を果たす間質因子・細胞を同定し、免疫抑制性微小環境に基づく膵癌サブクラス分類の提唱を達成する。 本研究では、細胞傷害性T細胞（CD8+細胞）の組織内密度を定量測定する画像解析技術を開発し、200例以上の膵癌症例で検証した結果、その大多数で腫瘍中心のCD8+細胞浸潤が辺縁に比して顕著に抑制されていることを客観的数値として示した。次いで、T細胞性免疫活性に加え、膵癌微小環境の持つ免疫抑制活性をin situで数値化する手法を確立し、これらの臨床病理学的意義を検証し、その結果を国際学会及び原著論文として発表した。さらに腫瘍中心と辺縁のCD8+細胞浸潤に大きな差のある症例につき、先行研究で報告されている多数の免疫抑制因子を検証する中で、CD8+細胞の腫瘍内浸潤抑制に関わる有力な候補として、膵癌特有の線維性間質に着目した。多重蛍光染色を応用した画像解析を用いて、複数の線維芽細胞サブタイプを同定することに成功し、これらの占有率やコラーゲン量を同一切片上で定量画像解析し、膵癌間質が3種の異なる間質組成タイプに亜分類されることを見出した。これらのうち、ある特定の線維芽細胞サブタイプに富む間質組成タイプでは、前述した免疫抑制活性の指標との相関が示され、強い免疫抑制活性で特徴づけられる間質タイプであることが示唆された（国際学会で発表済、論文投稿準備中）。この結果は、Science誌やPNAS誌などで報告されている膵癌モデルマウスを用いた先行研究結果を支持するヒト組織として初のエビデンスであり、免疫療法との併用を想定した間質標的治療の実現には、間質組成タイプに係る患者層別化が重要性であることが示唆される。 The cancer stroma-modifying treatment against pancreatic cancer has emerged as a promising strategy to enhance the response to immunotherapy. This study aimed to identify the stromal factors/cells that may play a central role in the suppression of adaptive T-cell mediated immunity in human pancreatic cancer and to propose the system of subtyping the immunosuppressive pancreatic cancer microenvironment for the development of individualized therapeutic strategies targeting cancer stroma against this highly heterogeneous disease. Firstly, we established computational quantitative system to calculate tumor-infiltrating CD8+ cells. Using this image analysis system, we analyzed 200+ human pancreatic cancers, and found that most tumors had substantially lower CD8+ cell density in the tumor center than in the tumor margin, to establish the quantitative evaluation methods for the anti-tumor immune activity and for the in situ immunosuppressive activity of pancreatic cancer microenvironment. Then, we clarified clinicopathologic significances of these quantitative measurements, and presented these results in international meetings and an original paper. We further examined candidate factors/cells contributing to the immunosuppression of pancreatic cancer stroma, and focused on the desmoplastic stroma. Utilizing multiplex immunohistochemistry-based image analyses, we successfully identified several fibroblast subtypes and quantified the amount of fibroblast subtypes in addition to intratumoral collagen fibers within pancreatic cancer tissues to identify three distinct stroma types. Among them, one stroma type was characterized by increased compositions of a specific fibroblast subtype (xCAF) and showed a significant correlation with aforementioned immunosuppressive activity of pancreatic cancer (manuscript in preparation). This is the first evidence in human specimens linking xCAF to antitumor T-cell immunity, which was suggested by previous studies (i.e. Science, PNAS) on murine models of pancreatic cancers. In conclusion, our study identified a stroma type closely associated with T cell immunosuppression, likely underscoring the importance of patient stratification according to stromal compositions for the development of novel stroma-targeted therapies to synergize with immunotherapies.
