• Note (General): type:text; 原発性アルドステロン症(PA)は、副腎皮質から過剰なアルドステロンが産生される疾患であり、潜在的な患者数は 300 万人に上ると考えられる。過剰なアルドステロンは高血圧のみならず、血管の炎症やリモデリングを引き起こし、PA 患者は高頻度に心血管病を合併する。しかし侵襲性の高い診断法が障壁となり、正確な罹患数は明らかでない。私達はこれまでに、患者副腎組織において新しいアルドステロン産生責任細胞を同定し、また特殊なステロイドを産生する事を見出した。本研究では、質量分析を駆使し、微量採血サンプルから、これらのマーカーステロイドを迅速に検出する技術開発を行う事を目的とした。 本研究開発により、患者副腎静脈からサンプリングされた微量血清(10uL)を用いて、マトリックス支援レーザー脱離法(MALDI)質量分析(MS)により、血中ステロイドを迅速に検出するプロトコルを開発した。極微量しか存在しないマーカーステロイドを高感度検出するために、ステロイド構造のケトン基に極性基付与する誘導体化試薬を活用した。これによりMALDI-MSにおける感度 は100 倍以上に向上した。さらに定量性を向上させる為に、安定同位体で標識したコルチゾール、アルドステロンを内部標準として添加した。開発したメソッドを用いて、西本紘嗣郎准教授(埼玉医大)と連携し、患者検体の血清解析を行った。左右の副腎静脈から得られる血清検体は、片側が腫瘍側、もう片方が健常であり、腫瘍側で数〜数十倍アルドステロンが高濃度に分泌されている事が、既存の臨床診断法により分かっている。今回開発したMALDI-MSを用いた方法でも、既存法と同程度のコルチゾール、アルドステロンの左右比を30分以内に得ることが出来た。 今後は、医療機器として多数の医療機関に既設の(医療機器として認定された)質量分析計で即座に実施可能なアプリケーションとして頒布を目指す。 Primary aldosteronism (PA) is an endocrine disease in which excess aldosterone is produced from the adrenal cortex. Its potential number of patients is thought to reach 3 million. Excess aldosterone causes not only hypertension but also vascular inflammation and remodeling, and eventually PA patients frequently suffer from cardiovascular disease. However, the exact morbidity is not clear because of the highly invasive diagnostic method for PA. We have previously identified a novel aldosterone-producing cell populations in the patient's adrenal gland and found that those also produce characteristic steroids, called hybrid steroids. In this study, we aimed to develop the technology for rapid detection of these marker steroids from micro blood samples by using mass spectrometry. Through this research and development, we have established a protocol for rapid detection of steroids in bloods by matrix-assisted laser desorption (MALDI) mass spectrometry (MS) using trace serum (10 uL) obtained from adrenal vein of patients. In order to detect with high sensitivity the marker steroids, those are present in a very small amount, we utilized a chemical derivatization reagent that attaches a polar group to the ketone group of the steroid structure. As a result, the sensitivity of MALDI-MS was improved 100 times or more. Using the developed method, we performed a serum analysis of patient samples in collaboration with Associate Professor Koshiro Nishimoto (Saitama Medical University). As results, within 30 minutes, we were able to obtain almost same results for cortisol and aldosterone relative concentrations as in the existing method, which requires much more analytical time and amount of serum(>2mL). In the future study, we will aim to distribute the protocol as a useful application that can be immediately implemented with introduced mass spectrometers (certified as medical equipment) at many medical institutions.
