• Note (General): type:text; 超高齢多死社会のわが国では、人生の最終段階における医療・ケアについて、本人の意思を尊重する動きが高まっている。厚生労働省は2018年3月、人生の最終段階における医療・ケアの決定プロセスに関するガイドラインにACP（Advance Care Planning）の概念を導入し、本人が事前に家族や医療・ケアチームと繰り返し話し合うプロセスを重視している。 しかし、クリティカルケア領域では容態の急変等により患者の意思確認が困難な場面も多く、代理意思決定を迫られる家族の心理的負担は重く、支援の重要性が高まっている。そこで、クリティカルケア領域における終末期代理意思決定の家族支援に関する文献を検討し、先行研究から現状と課題を明らかにすることを目的とした。 医中誌web版で「クリティカルケア」and「終末期」and「代理意思決定支援」のキーワードで検索を行ったところ10件が該当し、そのうち原著論文は3件であった。その他、関連学会のガイドラインや指針、雑誌の特集、研究報告等幅広く精読し、いずれの文献も概ね2014年以降に発表されたものであった。具体的内容としては、ICUにおける終末期ケアの現状や医療者の認識に関する質問紙調査、看護実践についてのインタビュー調査、ICUで終末期を迎えた患者家族のニードに関する調査等があった。 クリティカルケア領域に限定した終末期の代理意思決定支援について検討した結果、国内の先行研究は希少であるものの、ここ数年で医療者の関心は高まっており、看護実践や組織体制の実態、さらに、より良い支援を模索している現状が明らかとなった。 一方、課題としては、様々なガイドラインや指針はあるものの、7割以上の看護師が終末期の家族ケアに困難を感じており、その具体的な実情や支援を受けた家族の声を調査する必要性が明らかとなった。 In Japan, a super-aging and multi-death society, there is a growing movement to respect the will of the individual regarding medical care at the end of life. However, in the critical care field, there are many situations where it is difficult to confirm the patient's intention due to a sudden change in condition, etc., and the psychological burden of the family who is forced to make a surrogate decision is heavy, and the importance of support is increasing. The purpose of this study is to examine the literature on family support for end-of-life surrogate decision-making in the critical care field, and to clarify the current situation and issues from previous studies. The original papers, the guidelines of related societies, featured articles, and research reports, etc. were extensively read, and all of the documents were generally published from 2014 onwards. As a result of examining the end-of-life surrogate decision support limited to the critical care field, although previous researches in Japan are rare, medical professionals have been interested in the care last few years, and it has become clear that they are looking better support. On the other hand, as an issue, More than 70 percent of nurses have difficulty in family care at the end of life. A further study of the difficulty and the voice of the family who received the supports should be conducted.
