• Note (General): type:text; 長島型掌蹠角化症は、アジア人に多くみられる先天性の掌蹠角化症であり、日本にはおよそ 1 万人の患者が存在する。主な症状は掌蹠の潮紅を伴う過角化と多汗、悪臭である。本研究の目的は、 長島型掌蹠角化症において、手足に潮紅と悪臭を生じる病態原理を解明することである。 １．掌蹠の悪臭 スワブ法により手足の各部位別に細菌叢を取得し、細菌数の定量解析と、次世代シーケンサを用いたマイクロバイオーム解析を行った。細菌数を近似的に測定するために確立した16S rRNAコピー数の定量方法を用いて健常人と長島型掌蹠角化症患者での細菌数比較を行った。長島型掌蹠角化症の手足では有意に菌数が増加しており、マイクロバイオーム解析により、長島型掌蹠角化症で増加している主要な菌を同定した。外用剤Zの外用後は、菌数の減少とともに臭気の改善を認めた。 ２．掌蹠の潮紅と過角化 研究期間中に、炎症性疾患Xを合併している長島型掌蹠角化症の患者で、炎症性疾患Xに対して阻害薬Yを投与したところ、長島型掌蹠角化症の症状である手足の潮紅と過角化がほぼ消失したという症例に出会う幸運を得た。そこで、本症の皮膚において、そのシグナル経路が亢進しているか？亢進しているのであれば、どの細胞で亢進しているのか？について、患者生検検体を用いた検討を行った。結果、真皮浅層の血管内皮細胞において特異的に本シグナル経路が亢進していることを、複数のリン酸化特異抗体による染色で確認した。血管内皮細胞における本シグナル経路の役割はほとんど分かっていない。阻害薬Yの外用による治療の臨床研究へ進む予定である。 以上の研究を通じ、長島型掌蹠角化症における手足の悪臭に対して外用剤Zが有効であることを確認するとともに、手足の潮紅と過角化に阻害薬Y外用の有効性が期待できることを明らかにした。 Nagashima-type palmoplantar keratosis is a congenital palmoplantar keratosis that is common in Asians. There are approximately 10,000 patients in Japan. The main symptoms are erythema, hyperkeratosis and odor of the hands and feet. The purpose of this study is to elucidate the pathological principle of Nagashima-type palmoplantar keratosis, which causes erythema, hyperkeratosis and odor in the hands and feet. 1. Palmoplantar odor Bacterial flora was obtained by the swab method, and quantitative analysis of bacterial numbers and microbiome analysis were performed using a next-generation sequencer. We compared bacterial counts in healthy subjects and the patients using the quantitative method of 16S rRNA copy numbers established by us in the prior study. Microbiome analysis identified the major microorganisms that are up-regulated in Nagashima-type palmoplantar keratosis. After topical application of topical Z gel, an improvement in odor was observed along with a decrease in the number of bacteria. 2. Erythema and hyperkeratosis of the hands and feet During the study, we found a patient with Nagashima-type palmoplantar keratosis complicated by Inflammatory disease X. When the patient was treated with a Y-signal pathway inhibitor for the inflammatory disease X, the erythema and hyperkeratosis mostly disappeared within several weeks. Therefore, we compared the activation of Y-signal pathway between healthy subjects and Nagashima-type palmoplantar keratosis patients. We found that Y-signal pathway was specifically activated in the endothelial cells in the upper dermis by staining with several phosphorylation-specific antibodies. Although the role of the Y-signal pathway in vascular endothelial cells is largely unknown, we plan to move forward with clinical studies to treat Nagashima-type palmoplantar keratosis with topical Y-signal pathway inhibitors. Through these studies, we confirmed the efficacy of topical Z gel for the treatment of malodor in Nagashima-type palmoplantar keratosis and the efficacy of topical Y-signal pathway inhibitor for the treatment of the erythema and hyperkeratosis of the hands and feet.
