• Note (General): type:text; 厳しい財政状況と満たされていない医療ニーズの存在に直面している日本政府は、研究開発のインセンティブと薬価/薬剤費のコントロールの両方を優先して考える必要がある。しかし、研究開発のインセンティブを強化するために薬価を引き上げると、患者ならびに医療保険の財政的負担が増大する（日本の公的医療保険のシステムでは、ほぼすべての処方薬が償還され、薬価は政府によって決定およびコントロールされる）。 したがって、薬価/薬剤費を引き上げることなく、より高い研究開発インセンティブを達成する方法を見つけることが重要となる。そこで、本研究では、「革新的医薬品の研究開発インセンティブの維持・向上」と「薬剤費上昇抑制」の両立に向けた考察を行った。 第一に、「革新的医薬品の研究開発インセンティブの維持・向上」の必要性に関し、ドラッグ・ラグの現状を把握し、ドラッグ・ラグ（特に、開発ラグ）が再燃していないかどうかを検証した。分析の結果、日米間の比較により、開発ラグがまだ日本に存在することが明らかになった。また、開発ラグは、2010年度以前と比較して2012年度に有意に縮小したが、その後は縮小したとは言えなかった。最後に、またイノベーションを促進する政策のうち、「先駆け」および優先審査システムは開発ラグの縮小に有意に効いたが、画期性加算や有用性加算がついた医薬品の開発ラグが有意に小さいとは言えなかった。 第二に、リスク回避的な企業を想定し、薬価に関するリスクを低減することで、どの程度研究開発インセンティブが高まるかを推定する。検証の結果、プラスマイナス10%のリスクを減らすことは、5%の薬価プレミアムがあることと推定された。 Due to severe financial conditions and unmet medical needs, the Japanese government must prioritize both R&D incentives and drug prices/expenditures. However, increasing reimbursement drug prices to strengthen R&D incentives also increases the financial burden on patients and the National Health Insurance, wherein virtually all prescription drugs are reimbursed, with prices determined and controlled by the government. Hence, it is important to find ways to achieve higher R&D incentives without raising drug prices/expenditures and to balance R&D incentives for innovative drugs and control of drug expenditures. Firstly, this paper focuses on the development lag between Japan and the United States. We find that an analysis of the full distribution of development lag shows that a development lag behind the United States still exists in Japan. Second, we find that the development lag significantly decreased in FY 2012, compared with FY 2010 or earlier, but stopped decreasing thereafter. Last, some pro-innovation policies are found to significantly shorten the development lag, while others do not. For instance, the "Sakigake" and Priority Review systems significantly shorten the development lag, whereas increased premiums, such as Innovativeness and Usefulness Premiums, do not. Secondly, we focus on reimbursement drug price risk of pharmaceutical firms. We find that reducing price risk is effective to enhance R&D incentives. The simulated impact of removing a ±10% price risk is equivalent to winning a 5% premium, which is substantial.
