• Note (General): type:text; 5-アミノサリチル酸製剤(5-ASA)は潰瘍性大腸炎(UC)の軽症から中等症の症例に対する第一選択治療薬である。5-ASA製剤は小腸で吸収されずに大腸まで到達させて、大腸局所で効果を発揮させることが重要であり、臨床効果と大腸粘膜内の5-ASA濃度が関係することを我々はこれまで報告してきた。一方で5-ASAは大腸局所で吸収されたのち、代謝酵素であるN-acetyl-transferase(NAT)により、acetyl-5ASA (Ac-5ASA)に変換される。Ac5ASAは代謝産物であり薬効とは関連がないことより、大腸粘膜のAc-5ASA濃度と治療効果との関連はないことが考えられるが、我々の探索的検討では、大腸粘膜内の5-ASAおよびAc-5ASAの双方が臨床的活動度および内視鏡的活動度と関連があることが示されている。本研究では5-ASAを服用している患者における内視鏡活動度と、代謝産物であるAc-5ASAおよび代謝酵素であるN-acetyl-transferase(NAT)活性との関連を検討した。 5-ASA製剤を８週間以上服用しているUC患者を対象に大腸内視鏡を施行し、直腸およびS状結腸の内視鏡的活動度と大腸粘膜Ac-5ASA濃度との関連を検討した。その結果、S状結腸における内視鏡的寛解例は非寛解例に比して大腸粘膜内５―ASAおよびAc-5ASA濃度が有意に高いことが示された。一方で、直腸粘膜では内視鏡的寛解例の方が、非寛解例よりAc―５ASA濃度が高い傾向にあったが、有意差は認められなかった。さらに5-ASA製剤の中で大腸で緩徐に5-ASAが放出されるmultimatrix system 5-ASA服用患者においては、直腸粘膜濃度の方がS状結腸濃度より高いことが示された。以上より大腸Ac-5ASA粘膜濃度も５―ASA濃度と同様に治療効果と関連があることが示された。ドラッグデリバリーシステムによりS状結腸、直腸に5-ASAをより多く到達させることが治療効果の向上につながると考えられた。またNAT活性については測定系が安定せず、有意な結果を示すことができなかった。 5-aminosalicylic acid (5-ASA) is the first-line treatment for mild to moderate cases of ulcerative colitis. It is important that 5-ASA preparations reach the large intestine without being absorbed in the small intestine and have an effect locally in the large intestine. We have previously shown that the clinical effect is related to the concentration of 5-ASA in the colonic mucosa. On the other hand, 5-ASA is absorbed locally in the large intestine and then converted to acetyl-5ASA (Ac-5ASA) by the metabolic enzyme N-acetyl-transferase (NAT). Ac-5ASA is not related to clinical efficacy. Therefore, it is conceivable that there is no relationship between mucosal Ac-5ASA concentration and the therapeutic effect. However, our preliminary invesstigation indicated that both 5-ASA and Ac-5ASA have been shown to be associated with clinical and endoscopic activity. In this study, we investigated the relationship between endoscopic activity in patients taking 5-ASA and the activity of metabolite Ac-5ASA and metabolic enzyme N-acetyl-transferase (NAT). The relationship between endoscopic activity and mucosal Ac-5ASA concentration in the colonic mucosa was examined. The results showed that endoscopic remission in the sigmoid colon had significantly higher concentrations of 5-ASA and Ac-5ASA in the colonic mucosa than patients without endoscopic remission. On the other hand, Ac-5ASA concentrations in the rectum tended to be higher in patients with endoscopic remission than without remission, but no significant difference was observed. In addition, the rectal mucosal concentration was higher than the sigmoid colon concentration in patients receiving the multimatrix system 5-ASA, which releases 5-ASA slowly in the large intestine among the 5-ASA preparations. Our results indicated that the concentration of mucosalAc-5ASA was related to the therapeutic effect similarly to the concentration of 5-ASA. It was thought that the drug delivery system would allow 5-ASA to reach the sigmoid colon and rectum in greater amounts, leading to an improved therapeutic effect. The measurement system was not stable for NAT activity, and no significant results could be shown.
