• Note (General): 出版タイプ： VoR; type:text; 特定機能病院では未承認医薬品、承認済み医薬品の禁忌・適応外使用（適応外等）を把握し、その妥当性を確認することが義務付けられている。適応外等の実例はデータベース化されておらず、電子カルテ上の患者の病名とデータベースとを照合して適応外等を検出する事は不可能である。最新の AI (artificial intelligence) では過去数十年分の MEDLINE 収載論文の要旨を学習し、必要な情報を抽出することが可能である。そこで本研究では①AI を利用して文献やガイドラインから特定の薬剤と紐付けられる病名・症状名を抽出し、適応外等のデータベースを作成する。文献情報から適応外等の有効性・安全性に関わる情報を併せて抽出し、情報源のエビデンスレベルに基づいて投薬の妥当性を提示させる。②電子カルテから患者の年齢、性別、アレルギー・副作用歴、臨床検査値、病名、妊娠の有無、投与薬剤名等を抽出し、適応外等データベースと突き合わせて適応外等を検出する仕組みを構築することとした。本研究は3年の研究期間を予定しており、2020 年度はその3年目である。①については2019年度にプロトタイプを作成し、「医療上の必要性の高い未承認薬・適応外薬検討会議」で公表されている8医薬品について文献の抽出を可能とした。2020年度は日本語医薬品名・病名の表記ゆれを吸収して検索する仕組みを構築し、適応外等のデータベースを作成するためのトライアルを実施した。②についても2019年度に4種類の薬剤を対象にシステムを構築した。2020年度は検出アルゴリズムをブラッシュアップし、適応症の突合については実用レベルの性能を達成した。一方で禁忌の検出には電子カルテ入力のレベルで対応が必要となることを見出した。 Special functioning hospitals are obliged to take control of the use of unapproved drugs, contraindications and off-label use of approved drugs, and evaluate their relevance. There is no database of actual off-label cases, and it is impossible to detect off-label cases from the names of patients' diseases in the electronic medical record. The latest AI (artificial intelligence) can learn the abstracts of articles in MEDLINE for the past several decades and extract the necessary information. Therefore, in this study, (1) Extract diseases / symptoms associated with a specific drug from the literature and guidelines using AI and create a database of off-label uses. It also extracts information related to efficacy and safety of the of off-label use from literature, and provide validity based on the evidence level. (2) Extract the patient's age, gender, allergy, medical and side effect history, laboratory test, pregnancy status, administration drug, etc. from the electronic medical record, and construct a mechanism to detect off-label use. This research is planned to last three years, and 2020 is the third year of the research period. (1) A prototype was created in 2019, enabling the extraction of literature for eight drugs announced by "The Evaluation Committee on Unapproved or Off-labeled Drugs with High Medical Needs". In 2020, a trial was conducted to create a database for off-label use by building a search mechanism that absorbs notation distortions in Japanese drug names and disease names. (2) The system was also built for four drugs in 2019. In 2020, the detection algorithm was brushed up, and the system achieved a practical level of performance for collating indications. On the other hand, it was found that the detection of contraindications needs to be addressed at the level of electronic medical record entry.
